PRELIMINARY NOTIFICATION OF EVENT OR UNUSUAL OCCURRENCE-PNO-77-54 This preliminary notification constitutes EARLY notice of an event of POSSIBLE safety or public interest significance. The information presented is as initially received without verification or evaluation and is basically all that is known by IE staff on this date.

Subj ect:

REACTOR COOLANT PUMP STARTUP IN IDLE LOOP RESULTING IN REACTOR TRIP AND SAFETY INJECTION With the reactor at 25 percent power on April 12, 1977,following a startup, a Reactor Coolant Pump (RCP - one of four) tripped at 10:32 a.m. when power was lost to its 4160 V bus. Contrary to license requirements the reactor was operated with three RCPs for thirty minutes. The RCP, which had tripped, was subsequently started. Upon starting the RCP in the idle loop a turbine trip and reactor trip resulted from a high water level in a steam generator.

The safety injection system operated because of steam flow and reactor coolant temperature conditions.

NRC, Region I inspectors were dispatched to the site to evaluate the oc-currence.

It has been determined that the primary cause of the occurrence was personnel error due to unfamiliarity with a specific " license" condition.

All reactor systems responded properly. The reactor shutdown margin af ter -

Seas the reactor trip was well below Technical Specification limits.

Media interest is expected because of the close attention to operating problems at this facility. A press release is not planned.

NRC, Region I, (Philadelphia) received notification of this c,ccurrence by
