APPENDIX A NOTICE OF VIOLATION Maine Yankee Atomic Power Company Docket No.

50-309 Maine Yankee Nuclear Power Station License No. DPR-36 As a result of the Performance Assessment Team (PAT) inspection conducted from May 13-24 and June 3-7, 1985, and the followup inspection conducted November 5 to December 31, 1985, and in accordance with the NRC Enforcement Policy (10 CFR 2, Appendix C), the following violations were identified:

A.

10 CFR 50, Appendix B, Criterion XII requires that measures shall be estab-lished to assure measuring and testing devices usei in activities affecting quality are properly controlled, calibrated, and adjusted to maintain accuracy within necessary limits.

Maine Yankee Operational Quality Assurance Program,Section XII, " Control of Measuring and Test Equipment," Revision 2, and Maine Yankee Procedure 0-06-5,

" Measuring and Test Equipment," Revision 2, require similar control mechanisms for measuring and test equipment (M&TE).

Contrary to the above, (1) calibration procedures for outside micrometers and torque wrenches and calibration cross-check procedures for dead weight testers were not adequate in that these procedures did not designate the calibration points to be checked, (2) evaluations were not conducted to verify the valid-ity of tests performed with pieces of M&TE that were later discovered out of tolerance, and (3) M&TE usage was not always identified on usage cards as required by licensee administrative procedures.

This is considered to be a Severity Level V violation.

Your response to this item need only address corrective action for item (3), M&TE usage cards, since corrective actions for the other items have been reviewed and found to be adequate.

B.

Maine Yankee Operational Quality Assurance Program,Section II.C.13, states that the licensee will comply with the requirements of ANSI N45.2.11-1974,

" Quality Assurance Requirements for the Design of Nuclear Power Plants." ANSI N45.2.11-1974, Section 4.2, requires that design analyses, such as physics, stress, thermal, hydraulic, and accident, shall be performed in a planned, controlled and correct manner.

Contrary to the above, the licensee incorrectly calculated the load capabili-ties of an anchor bolt installation associated with EDCR 83-07, " Modifications of Feedwater Line Supports H-15."

This is considered to be a Severity Level V violation.

Based upon the results of this inspection, it appears that you have adequately implemented corrective actions and therefore no response to this item is required.

10 CFR 50, Appendix B, Criteria VI, requires that measures be established to l

control the issuance of documents such as instructions, procedures and draw-ings including changes thereto which prescribe all activities affecting qual-ity.

Maine Yankee Procedures 0-01-2, " Drawing Control," and 17-22-3, " Drawing Up-date," require (1) that uncontrolled drawings be destroyed upon completion of an activity, or that uncontrolled drawings be periodically reverified if they are used for a prolonged period of time, and (2) that controlled drawings be stamped to reflect outstanding drawing changes due to completed design changes.

Contrary to the above, (1) two sets of uncontrolled drawings were kept in the control room along with controlled drawings, and were not verified as required, and (2) controlled drawings reviewed at four locations did not reflect the same status of outstanding engineering design change requests (EDCRs).

This is considered to be a Level V violation.

Your response to this item need only address item (1), uncontrolled drawings, as corrective actions for item (2) have been reviewed and found to be adequate.

D.

Technical Specifications, Section S.8.1, requires that written procedures covering the applicable procedures recommended in Appendix "A" of Regulatory Guide 1.G3, " Quality Assurance Program Requirements (Operation)," November 1972, be established, implemented, and maintained.

Regulatory Guide 1.33 and Maine Yankee Procedures require that shelf lives for reagents and standards be verified as having not elapsed prior to use and provides for remedial action should it be necessary to use reagents or stand-ards with an expired shelf life.

Additionally, they require that procedures be established for the calibration of laboratory equipment.

Contrary to the above, (1) several chemicals with e' pired shelf lives were x

found to be in use on or about May 16, 1985, with no remedial action having been taken, and (2) no calibration procedure was available for the Nuclear Measurements Corporation gas flow proportional counter used in the radio-chemistry laboratory to establish the allowable flow rate for liquid effluent releases.

This is considered to be a Severity Level V violation.

Based upon the results of this inspection, it appears that you have adequately implemented corrective actions and therefore no response to this item is required.
