SUMMARY

On December 13, 1985, representatives of Mississippi Power and Light Company (MP&L) met with the NRC at MP&L's request, in the Regional office in Atlanta, Georgia.

The topic of discussion was the November 6,1985, Emergency Diesel Generator overspeed event at Grand Gulf with respect to the contributing cir-cumstances and corrective actions taken/ lessons learned.

MP&L commenced their presentation by confirming that the Division 1 Emergency Diesel Generator was being tested per a Special Instruction following preventive maintenance which had required draining the oil from the governor.

After a successful start, the engine surged to over.600 RPM causing extensive engine damage before it could be shutdown.

From their investigation, the licensee concluded that the cause of the overspeed was the governor not being properly filled with oil and inadequate overspeed protection for this type event. Conse-quently, the licensee has taken the following corrective actions:

(1) Revised the governor fill and vent procedure to include air rolling the diesel and refilling the governor until no level decrease is observed after air rolling.

(2) Revised the existing operating instruction to require: checking of the governor oil level prior to manual' starts; the performance of an overspeed timing check within 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> of starting the diesel; stationing of an operator at the manual push-pull shutdown switch; and reducing the hydraulic governor load limit to 40% in order to limit acceleration after governor maintenance.

(3) Conducted training of diesel mechanics and operators to make them aware of the potential for an overspeed event such as this and the precautions /

actions to be taken in order to prevent it.

Additionally, the licensee indicated that the diesel manufacturer, Transamerica DeLeval Incorporated (TDI), has proposed modifications to reduce fuel rack and air strangulation valve shutoff times from 4-6 seconds to 1.0 second. Pending final MP&L " technical review and acceptance, these modifications will be implemented.

Although not considered as contributing factors to the overspeed event, MP&L also identified the use of Special Instructions and maintaining vendor manuals c

up-to-date as areas requiring improvement.

Unlike the Standard Operating Instruction, the Special Instruction being used during this event did not require the governor speed control to be reduced before starting the diesel. Accordingly, the licensee stated that this Special Instruction, which was written by a foreman, received an inadequate enaineering review.

Consequently, Special Instructions for major rotating equipment will now require a maintenance engineering signoff and an operations management review and signoff.

Also, where possible, Special Instructions will reference /make use of applicable Standard Operating Instructions.

Regarding vendor manual updating, MP&L agreed that their Woodward Governor technical manual did not reflect the latest revision. However, they stated that even the latest revision inadeauately addressed the filling of the governor, and would not have prevented the overspeed event.

MP&L contacted all appropriate subvendors to TDI in order to obtain updated information, and is working with the TDI owners group and INP0 to ensure the industry is aware of the event and associated findings.
