Gentlemen:

This refers to the special safety inspection conducted by Messrs. T. M. Tongue

and T. E. Taylor of this office from March 14 through March 25, 1988, of

activities at the Braidwood Station, Unit 2, authorized by NRC Operating

a

License No. NPF-75, and to the discussion of our findings with

Mr. R. E. Querio and others of your staff at the conclusion of the inspection.

This special inspection was conducted upon your identification of an event on

March 13,1988, in which the Unit 2 "B" Safety Injection pump manual discharge

valve was found locked shut, thus rendering the pump inoperable for over seven

days. The enclosed inspection report identifies the areas examined during the

,

inspection with respect to the facts and circumstances related to the event.

The inspection consisted of an examination of relevant procedures, records,

observations, and interviews with personnel.

During this inspection, certain of your activities appeared to be in violation

-

of NRC requirements, as specified in the enclosed Notice. A written response

is required.

.

During the review of this violation, we considered escalated enforcement

action. NRC Region III concluded that the circumstances surrounding the

event, including reactor power levels, pcwer history, and the availability

of redundant equipment, caused the event to be less significant than if it

had occurred under other circumstances. Therefore, we concluded that the

significance of the event warranted a Severity Level IV violation.

We are concerned about the factors which caused or may have contributed to

the event, such as quality and formality of communications, control of locked

equipment and related training in these areas.

In addition to your response

to the enclosed Notice of Violation, please address these topics and your

actions to prevent additional events resulting from similar causes.

In accordance with 10 CFR 2.790 of the Comission's regulations, a copy of

this letter, the enclosures, and your response to this letter will be placed

in the NRC Public Document Room.
