Gentlemen:

Subject: Inspection Number 50-293/88-20

This letter refers to the special inspection conducted by Mr. H. J. Kaplan of

this office on May 2-6, 1988 at the Pilgrim Nuclear Power Station, Plymouth,

Massachusetts and in the Region 1 office the week of June 6,1988. The purpose

of the inspection was to determine if the licensee had modified the Pilgrim

Plant containment in accordance with the commitments to NRC concerning Unre-

solved Safety Issue (USI) A-7. Mr. Kaplan focused his attention on the QA

aspects of the modification involving materials, welding, and NDE.

At the inspection exit meeting on May 6, 1988, Mr. Kaplan informed

Mr. K. Highfill of your staff that he was unable to verify with complete

certainty that the modification work performed by Graver Energy Systems, the

, principal installer, was in complete agreement with the specified requirements;

Although extensive modification records existed in the form of microfiche, key

documents that were necessary to correlate the records to specific hardware

installed in the plant were not available. Mr. glan requested that your

staff retrieve the key documents, which apparencly had not been microfilmed and

assemble the records to permit a meaningful. assessment. Subsequently,'on

June 8,1988, Mr. P. Hamilton and Mr. M.- Andrews of your staff presented

collated document packages to Mr. Kaplan for his review at our Regional Office

in King of Prussia. Mr. Kaplan conducted his review of the submitted document

packages and concluded the records satisfactorily demonstrated that the modi-

fication of the Mark I Containment had been performed in accordance with the

specified installation requirements.

Wi".hin the scope of this investigation, no violations were observed; however,

we do want to emphasize that records of' activities that could affect plant

quality, such as plant modifications, should be readily retrievable.
