Exit Interview

The inspection scope and findings were summarized on March 24, 1988, with

the Operations Superintendent and members of his staff.

The licensee acknowledged the findings and took no exceptions.

The

licensee did not identify as proprietary any of the materials provided to

or reviewed by the inspectors during this inspection.

3.

Licensed Operator Accelerated Requalification Training Program

On March 22-24, 1988, three inspectors from the Regional office inspected

the Accelerated Requalification Training Program as applied to licensed

operators in Group 4.

The program was conducted in the Training Facility

at the plant site. It consisted of both classroom and simulator training;

'the entire program (consisting of four groups) was implemented in early

1986 in response to the poor performance of the operators on.the Requali-

fication Examination administered by Regional examiners in November 1985.

The - inspection included confirming that training commitments were met,

reviewing written exams, reviewing simulator exam documentation, and NRC

regrading of selected written exams to ensure that licensed operators in

Group 4 had satisfactorily completed the training program and are ready

to perform licensed duties in the shutdown and refueling modes only,

until the NRC gives them requalification exams using the new methodology.

The new requalification exams will be given prior to Unit 2 startup.

The inspection team recommended to the Office of Special Projects manage-

ment that the 17 operators in Group 4 who had passed the TVA written and

simulator exams be allowed to perform licensed duties in the shutdown and

refueling modes.

The above also applies to an SR0 candidate in the hot

license class, who completed his training in March 1988.

The above _ inspection team recommendation was based on 'the following

inspection- team . observations, while using the guidance provided in

NUREG-1021, Operator Licensing Examiner Standards.

The inspectors reviewed the operator and senior. operator written exams and

. determined that the exams adequately covered the areas of principles of

nuclear plant operations for thermodynamics, heat transfer and fluid flow;

plant design; instrumentation and controls; plant operating procedures,

including normal, abnormal, emergency, and radiological control procedures;

and facility management controls.

The inspectors determined that the TVA grading of the written exams was

satisfactory by using the NUREG-1021 guidance of plus or minus 2 percent

allowance for regrading.

The selected sample of 50 percent regrading

included failing as well as passing grades (i.e. , 70 percent for each

,

category and 80 percent overall).

The inspectors reviewed the simulator exam documentation, along with

instructor documented comments to determine that the operators appeared to

perform satisfactorily during the simulator exam exercises.

The inspectors also reviewed the commitments addressed in the TVA Browns

Ferry Nuclear Performance Plan, Volume 3 related to operator training with

the following result:

a.

Completed commitment for increased training emphasis on thermo-

dynamics heat transfer, fluid flow, reactor physics, mitigating core

damage, and plant ar.d reactor transient analysis.

b.

Completed commitment for comprehensive weekly examinations followed

by a comprehensive final examination.

c.

Completed commitment to utilize emergency operating instructions in

simulator training.

d.

Completed commitment to not use uncontrolled reference material

during training on the simulator.

(Open) Com.mitment to improve communication between operators durirg

simulator exercises was still identified and documented as an TVA

instructor concern during the Group 4 February 1988, simulator exams.

Completed commitment for senior operators to effectively use emer-

gency operating instructions during simulator exercises.

simulator exercises on emergency operating instructions for licensed

operators as part of requalification upgrade training.

procedures and drawings are used during simulator exercises.

during simulator exercises.

This still appears to be a

concern as identified in e, above.

(0 pen) Af ter May 1988, the Browns Ferry simulator will have 42-46 modi-

fications made to make it similar to Unit 2 (instead of Unit 1) and at

that time the upgraded and revised Unit 2 procedures (emergency operating

instructions, general operating instructions, and plant operating instruc-

tions) will be used for simulator training instead of Unit 1 procedures.

The inspectors are concerned that all the operators may not receive

training on the ~ updated Unit 2 simulator and . revised Unit 2 procedures

prior to Unit 2 restart. This item will be tracked as Inspector Followup

Item (IFI 259,260,296/88-08-01).

The above i tems identified as open will be followed up i n future

inspections.

Based on the above NRC recommends that TVA utilize the 17 operators from

Group 4 (and one operator from the hot license class of March 1988) who

had completed the TVA Accelerated Requalification Program, and that they

be allowed to perform licensed duties in the shutdown and refueling modos

only, until the NRC can administer requalification exams which are to La

given before the Browns Ferry Unit 2 restart.

The operators are

identified by Docket numbers in the Attachment.

No violations or deviations were identified in the areas inspected.

The licensee stated that the Institute of Nuclear Power Operations and the

TVA Nuclear Managers Review Group are going to perform a training audit in

April 1988, and that the TVA Operational Readiness Review Team are to

perform a training audit in July 1988.
