., 1 APPENDIX A NOTICE OF VIOLATION Gulf States Utilities Docket: 50-458 River Bend Station License: NPF-47 During an NRC inspection conducted on May 1 through June 30, 1988, a violation of NRC requirements was identified. The violation involved an inadequate shift turnover. In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1988), the violation is' listed celow:

Inadequate Shift Turnover River Bend Station Technical Specifications, paragraph 6.8.1 a. requires that. procedures for activities identified in Appendix A of Regulatory Guide 1.33, Revision 2, February 1978, be established, implemented and reintained. Regulatory Guide 1.33, paragraph 1.g, requires that admiriNtrative controls for shift and relief turnover be covered by written p ucedures.

Operations Support Procedure OSP-0002, "Shift Relief and Turnover,"

Revision 2, paragraph 3.?.2, states that each individual shall be responsible for reviewins and understanding the logs and checklist -

applicable to his shift position before assuming the shift. Attachment 3, "At-The-Controls (ATC) Oprator Relief Checklist," requires that the on coming ATC operator revie, the annunciator status and perform a board walkdown prior to assuming che shift.

Contrary to the above, on June 14,1988,at7:50a.m.(CDT)theNRC inspector identified that the ATC operator was not cognizant as to why annunciator P863-75A-C02, "Fuel Building Supply Air Low Flow," was lit.

This annunciator had been lit from the time the ATC operator assumed the shift at approximately 6:00 a.m. (CDT).

This is a Severity Level IV violation. (SupplementI)(458/8813-01)

Pursuant to the provisions of 10 CFR 2.201, Gulf States Utilities is hereby required to submit to this office within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including for each violation: (1) the reason for the violation if admitted, the corrective steps which have been taken and the results achieved, the corrective steps which will be taken to avoid further violations, and the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
