e SEQUOYAH NUCLEAR PLANT UNIT 1 AND UNIT'2 SPECIAL REPORT 88-08 DESCRIPTION OF EVENT With unit 1 in mode 5 (0 percent power, 6 psis, 130 degrees'F) and unit 2.in mode 3 (0 percent power, 1700 psig, 450 degrees F), a fire breach permit issued on March 1, 1988, for fire door C-14 (to auxiliary Instrument room, unit 1, Control Building elevation 685) exceeded the 7-day limit. allowed by Techn! cal'Speelfication (TS) Limiting Condition for Operation (LCO) 3.7.12.
The breach permit expired on. March 8, 1988. The fire breach was issued to keep the fire door open due.to a broket door triob. The door was capable of being closed and latched but could not be opened easily without the knob.
Normal access to the Control Building including the main control room is through C-14 door and a high volume of traffic passes through the door daily, which required the door to be kept open, l
CAUSE OF EVENT The replacement door knob was not ave 11able as a stock material. The replacement door knob was purchased on an urgent basis but the vendor supplied an incorrect knob. The correct knob could not be obtained within the 7 day limit allowed by action statement (a) of LCO 3.7.12.
ANALYSIS OF EVENT I
This event is reported in accordance with the requirements of action statement (a) of LCO 3.7.12.
A continuous fire watch was established at the l
door. The existing fire detection and fire suppresalon system for the l
auxiliary-instrument room were operable and would have actuated in the event of a fire. Therefore, there was no danger to the safety-related requirement located in the room.
(.
CORRECTIVE ACTION l
A continuous fire wctch was immediately estabilshed at the door and was maintained until the door knoh was replaced, and the fire door returned to functional status on March 11, 1988. Procurement requests will be initiated to stock necessary parts to maintain fire doors.
