APPENDIX A NOTICE OF VIOLATION Houston Lighting & Power Company.

50-499 South Texas Project, Unit 2 Construction Permit:
CPPR-129 During an NRC inspection conducted on May 16 through June 17, 1988, a violation of NRC requirements was identified. The violation involved failure to identify nonconformances promptly.
In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix-C (1988),
the violation is listed below:
Criterion XV of. Appendix B to 10 CFR Part 50 states, "Measures shall be established to control materials, parts, or components which do not.
confonn to requirements in order to prevent their inadvertent use or installation. These measures shall include, as appropriate, procedures for identification, documentation, segregation, disposition, and notification to affected organizations. Nonconforming items shall be reviewed and accepted, rejected, repaired, or reworked in accordance with documented procedures."
The Quality Assurance Program Description for the Design and Construction of the South Texas Project Revision 21, dated February 18, 1988, paragraph 15.0, requires that component nonconformances be documented and resolution determined prior to initiation of the preoperational test program on the item.
Contrary to the above, on June 14, 1988, it was identified that an eddy current inspection on April 17, 1987, of Steam Generator No. "C", tube row 35, column 49, found a nonconformance, namely a bulge, that was not identified on a nonconformance report and resolved prior to preoperational testing. Additional review by the licensee of the same, data on all steam generators found nonconformances in four more tubes, namely overexpansions, which had not been previously identified on a nonconformance report.
This is a Severity Level IV violation.

letter transmitting this Notice, a written statement or explanation in reply, including for each violation:
(1) the reason for the violation if admitted, the corrective steps which have been taken and the results achieved, the corrective steps which will be taken to avoid further violations, and the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
