Inspection Report No. 50-271/87-16

This rebrs to the routine resident safety inspection conducted by Mr. G. Grant

and others of this office on August 1 - October 30, 1987, at the Vamont Yankee

Nuclear- Power St nion, Vernon,' Ver1nont. The inspection consisted of observa-

tion of activities, interviews with personnel and document reviews. The results

of the inspection are described in the NRC Region I Inspection Report enclosed

with this letter and were discussed with Mr. J. Pelletier of your staff at the

conclusion of the inspection.

This inspection was conducted during regular and backshift periods by five

resident and regional inspectors over a three-month period that included pre-

parations for, conduct of and startup from refueling / maintenance outage XIII.

No safety-significant concerns were noted as a result of their comprehensive

inspections. Licensed operator and non-licensed personnel performance, at all

levels and within all departments, and particularly plant and outage management

performance, was considered superior throughout the inspection period.

Based on the results of this inspection, it appears that one of your activities

was not conducted in full compliance with NRC requirements, as set forth in the

Notice of Violation enclosed herewith as Appendix A.

The violation concerns

failures to meet Technical Specification 6,5.8 requirements for radiation

control procedures, which establish controls and set limits on the release of

radioactive material from controlled areas. This violation has been categorized

by severity level in accordance with the "General Statement of Policy and

Procedure for NRC Enforcement Actions", 10 CFR Part 2, Appendix C.

You are

required to respond to this letter and, in so doing, should folluw the

instructions in Appendix A.

Section 3.4 of this report details the history of a violation brought to your

-

attention in our letter dated February 6,1987 (Inspection Report 86-25) and

the Notice of Violation enclosed therewith, concerning a failure to comply with

'

inservice testing requirements of ASME Section XI.

As described, the - NRC

Office of Nuclear Reactor Regulation has reaffirmed its position on this issue

which indicates that our initial assessment was proper and will therefore

stand.

The corrective actions detailed in your letter of March 12, 1987, in

response to the Notice of Violation, require revision to ensure future regula-

tory compliance on this issue. Please review this matter and respond in writ-

ing within 30 days detailing your additional measures or actions.

We have received your September 10, 1987 reply to the Notice of Violation,

brought to your attention in our letter dated August 21, 1987 (Inspection

Report 87-12), concerning non-conservative trip setpoints in - the Toxic Gas

Monitoring System. Your corrective actions appear responsive to our concerns.

Further evaluation of the long-term effectiveness of these actions will be

,

accomplished during routine inspections.
