SUBJECT:

SPENT FUEL RACKS FABRICATED BY U. S. TOOL AND DIE, IhCORPORATED, AND ITS PREDECESSOR In a letter dated August 24, 1987, we transmitted to you our findings based on an inspection of the U. S. Tool and Die facilities in Allison Park and Glenshaw, Pennsylvania. During this inspection, we found that implementation of the U. S. Tool and Die QA program failed to meet certain NRC requirements. The most serious of.these appeared to be a breakdown in the QA/QC program concerning in-process examination and weld inspection, resulting in cracked or missing welds in spent fuel racks. Because the predecessor of U. S. Tool and Die, Wachter Associates, Incorporated (WAI), fabricated the spent fuel racks used at Point Beach, we requested various information regarding the extent to which the U. S.

Tool and Die QA/QC program was relied upon to assure rack quality.

You responded to our request in a letter dated October 20, 1987.

In your letter you stated that the vendor QA programs were not solely relied upon to assure the quality of the spent fuel racks supplied to Point Beach.

Instead, Wisconsin Electric was 6ctively involved in all aspects of rack construction including participation in design review; audits of engineering and fabrication; review and approval of fabrication, weiding, cleaning and packaging procedures; and numerous source and receipt inspections. Wisconsin Electric perfonned inspections to verify the use of proper materials, dimensional / drawing conformance, and weld quality.

Furthermore, all racks were receipt-inspected at Point Beach.

You concluded that your extensive overview of the design and fabrication of the racks assured the adequacy of the racks provided to Point Beach.
