TOOL AND DIE AND ITS PREDECESSOR POINT BEACH NUCLEAR PLANT This letter is in' response to'your letter dated August 24, 1987, requesting information concerning the fabrication of spent fuel pool storage racks at U.
S. Tool and Die facilities.
This information addresses our purchase of spent fuel racks for Point Beach Nuclear Plant and the measures we employed to assure the quality of this product.
l Spent fuel storage racks for Point Beach were procured from Wachter Associates, Incorporated (WAI) under Purchase Order A-02007-S dated December 12,_1977, and associated changes.
Design and engineering of these racks were performed by WAI, with actual fabrication of the racks being subcontracted by WAI to U. S. Tool and Die (USTD) in Allison Park, Pennsylvania.
At the time the Point Beach racks were procured, WAI and USTD were separate and distinct companies.
Subsequently, as we understand it, USTD and WAI were consolidated and now operate as-U. S. Tool and Die.
In addition to this change in corporate structure, we further understand that extensive personnel changes have also occurred since the fabrication of the Point Beach racks.
Many of these changes were in the quality assurance area, including assignment of a new QA Manager.
At the time of rack procurement, the QA/QC programs of both WAI and USTD were evaluated by Wisconsin Electric Quality Assurance personnel.
With the exception of a few minor discrepancies, the programs were found to be acceptable.
The vendor QA programs were not solely relied upon to assure the quality of racks supplied for Point Beach Nuclear Plant.
To assure further rack quality, Wisconsin Electric closely monitored and became involved in the design and fabrication of the racks.

.This involvement included participation in design review meetings;faudits of engineering and fabrication; review and approval of fabrication, welding, cleaning, and packaging procedures;.and numerous source and receipt inspections.
Records documenting these reviews and audits are available for your inspection in our offices.
On-a regular basis, Wisconsin Electric personnel conducted audits and/or inspections of rack fabrication at USTD.
These activities were performed to assure that all quality and technical specifications were met. Lour inspections verified the use of. proper materials, dimensional / drawing conformance, and weld quality both.in process and prior to shipment.
In addition, all racks-were thoroughly' receipt-inspected at Point Beach.- As a result"of the inspections, only one potentially significant deficiency was identified.
This involved identification of slightly undersized welds when joining some of the sheet metal components.- The deficiency was subsequently resolved by an engineering evaluation which was conducted by WAI and reviewed and accepted by Wisconsin Electric.
No recurrence of this problem was noted during subsequent fabrication.
Based on our review, we have concluded that USTD performance I
during fabrication of the Point Beach spent fuel storage racks was acceptable.
Additionally, we performed extensive overview of design and fabrication activities to assure the adequacy of j
the racks provided.
We believe that an underlying cause of i
current USTD problems may be a result of the extensive changes in structure and personnel which have occurred since fabrication of the Point Beach racks.
No further Wisconsin-Electric actions are anticipated.
