NRC INTEGRATED INSPECTION REPORT NOS. 50-369/97-09 AND
50-370/97-09 AND NOTICE OF VIOLATION
Dear Mr. Barron:
On June 28. 1997, the Nuclear Regulatory Commission (NRC) completed an
inspection at your McGuire 1 and 2 reactor facilities.
The enclosed report
presents the results of that inspection.
During the six-week period covered by this inspoction, your conduct of
activities at the McGuire facilities was generally characterized by safety-
conscious operations sound engineering and maintenance practices, and careful
radiological work controls.
Good operational control was maintained on both
Unit 1 and Unit 2 power manipulations to support required maintenance, as well
as reduced inventory operations.
Conservative decision making was evidenced
throughout the resolution of increased primary to secondary leakage on the
Unit 2 A steam generator.
However, a violation of NRC recuirements was identified and cited in the
enclosed Notice of Violation (hotice).
Described in detail in the enclosed
report this violation concerns an inadvertent Engineered Safety Features
actuation due to an inadequate emergency diesel generator load sequencer test
procedure.
The report also identified an example of incomplete root cause
reviews on secondary components which challenged operators and required power
manipulations to correct.
In addition, one example of inappropriate control
of vendor personnel was identified during the period as detailed in the
report.
Please note that you are required to respond to this letter and should follow
the instructions specified in the enclosed Notice when preparing your re-
sponse
The NRC will use your response in part, to determine whether further
enforcement action is necessary to ensure compliance with regulatory
requirements.
