Today is Shift 2 on Saturday May 16th.
Core alterations were suspended for three days, (Wednesday shift 1 through Friday).
That work has been completed and the second fuel shuffle is to resume.
Control Rod K-5 has no position indication due to Instrument Maintenance replacing the RPIS probe.
The Mode switch is locked in REFUEL.
All control rods are fully inserted.
The Reactor cavity is flooded.
Secondary Containment is operable.
Communications have been established and tested satisfactorily earlier in the shift.
The following core alteration surveillances were completed satisfactorily:

Control Rod K-5 has no position indication due to Instrument Maintenance replacing the RPIS probe.

Unit 1 was operating at 100% rated power when a transient occurred. The Emergency Plan was activated and a Site Area Emergency (FS1) was classified at time 1330 due to high Drywell radiation of 700 R/hr.
At 1500 hours0.0174 days <br />0.417 hours <br />0.00248 weeks <br />5.7075e-4 months <br />, a General Emergency (FG1) was declared due to an UNISOLABLE leak in the HPCI Room. The following conditions are also present:
o RPV pressure is 750 psig and lowering.
o U1 HPCI Room radiation level is reading 2000 mR/hr and room temperature is 185 deg F and rising.
o MO 1-2301-4 and MO 1-2301-5 valves have lost position indication. An EO and Maintenance Team from the OSC have been dispatched to close the MO 1-2301-5 valve.
o Reactor Building D/P is -0.1 Inches.
o Main Chimney Release Rate is 2.81 E+05 µCi/Sec.
The EO Shift Communicator is performing Emergency Response Organization (ERO) duties.
At time 1600, the STA reports the following:
o RPV pressure is 400 psig and lowering.
o Drywell radiation level is 680 R/hr.
o The Operations and Maintenance team are continuing efforts to close MO 1-2301-5 valve.
o U1 HPCI Room radiation level is 1000 mR/hr and room temperature is 160 deg. F.
o Main Chimney Release Rate is 4.30 E+04 µCi/Sec.
o A storm front has moved in and the Wind Direction has shifted to 110 deg. Wind speed has changed to 4.5 m/s (10 mph).
THIS JPM IS TIME CRITICAL.
INITIATING CUE As the Shift Emergency Director, determine if any notifications to State and Local agencies are required. If so, prepare the necessary forms that would allow the Shift Communicator to complete the required State and Local notifications, (NARS).
Provide examinee with: Access to all Emergency Plan procedures as found in the simulator.
