HIGHLIGHTS OF WEEKLY INFORMATION REPORT WEEK ENDlNG FEBRUARY 5, 1988 M Authorization For Sequoyah Heatup At approximately 9:15 a.m., on February 4,1988, the Director of the Office of Special Projects released Sequoyah Unit 2 from the first of five pre-established NRC Hold-Points to allow plant heatup for testing and shakedown activities. Sequoyah has been shutdown since August 1985. The shakedown heatup, which is currently scheduled to last approximately 5 to 6 weeks, will allow both the NRC and TVA to monitor operator proficiency and effectiveness of TVA organizational changes and to identify any additional equipment problems that may arise. After this heatup, TVA will eitht:r cool down Sequoyah Unit 2 to correct identified deficiencies or they may keep the ,

Unit heated up awaiting Commission approval for restart.

An augmented NRC inspection plan which includes control room shift coverage has been put in place in accordance with the Sequoyah Restart Plan. An OSP manager is on-site to direct NRC restart coverage and daily site briefings are being provided to selected headquarters and regional staff.

PN-0SP-88-001 has been issued and will be updated appropriately.

Incident Respense Branch On February 3, the Director, Deputy Director, AE0D and staff members met with the Deputy Assistant Secretary for Energy Emergencies and the Director of the Office of 011 and Gas, Department of Energy (DOE), to discuss NRC's involvement in the Continuity of Government program and to establish a working relationship for implementing a mutually satisfactory program. General agreement was reached on the manner in which NRC would support DOE in its overall responsibilities to assess the 'J.S. energy situation and take appro)riate actions in the national interest. In addition, it was agreed that 30E would:

(1) keep NRC informed of relevant issues that may require NRC involvement in the national emergency preparedness program being developed by the Federal EmergencyManagementAgency(FEMA),and(2)provideNRCwithguidancefor  ;

developing and implementing that portion of the NRC national emergency preparedness program in which NRC will provide support to DOE. l West Valley Demonstration Project "-

The staff has received a report from the Department of Energy on the June 19, 1987, radiological contamination incident at West Valley. A subcontractor worker's hand was contaminated while handling a cable in a riser above the high-level waste tank. The area had been considered free of contamination, so the worker was not surveyed when he left his work station. The contamination was subsequently spread to several radiologically uncontrolled areas. Ten additional workers received skin contamination before two of them set off alarms at the main gate portal monitor. Most of the eleven workers with skin contamination, plus two additional workers, received slight internal contamination as well. The estinated 50-year dose connitment to the most highly contaninated individual is about 30 mrem. No contamination was carried beyond the site boundaries by this incident.

Second Building The Montgomery County hearing to determine whether to approve the developers' plans for the second building at the White Flint site has again been postponed, probably until later this month. Representatives of the developers have been meeting with Planning Board staff to resolve problems ,

which could delay the approval, l

l ITEMS OF INTEREST Office of Nuclear Reactor Regulation Week Ending February 5, 1988 NRR

1. Calvert Cliffs 2 At 4:46 p.m. on February 1,1988, with the unit at 100% power, the plant incurred an electrical fire in a power supply panel in the cable spreading rocm resulting in the loss of all control room annunciators for Unit 2. The fire was initially put out within 15-20 seconds by the Halon system. However, the fire restarted when the panel was opened by operators sent to investigate the cause of the fire. The panel was deenergized and the fire put out by C02.

Due to the sustained loss of annunciators, the licensee declared an Alert in accordance with its Emergency Plan at 5:46 p.m. All notifications were made as required by the alert declaration.

The licensee elected to kaao the unit at 100% power while assessing the situation. All control room indications were functioning normally, the plant computer was available to provide alarm printouts, and all .

emergency systems were available.

The operators were able to isolate the affected portion of the circuit which provides the audible alarms in the control room and reenergize the portion of the circuit providing visual alarms in the control room. The Alert was terminated at 7:10 p.m.

1 The Resident Inspector and a Region I specialist inspector were onsite and maintained telephone coverage from the Control Room with Region I and the Operations Center throughout the event. Region I dispatched two additional persennel to Calvert Cliffs during the evening. Additional ,

Region I and NRR personnel were dispatched to the site on the morning of l February 2, 1988 to monitor the licensee's troubleshooting and repair activities and to determine any generic implications of the event. This event is similar in nature to the Alert declared at Beaver Valley Unit 2 ,

on January 28, 1988 as a result of sustained loss of annunicators cause l by a fire in a remote annunciator cabinet. l

2. H. B. Robinson 1

H. B. Robinson began shutting down from 100% power on January 29, 1988 due to an uranalyzed DC power distribution condition. Specifically, a single failure of one of the two emergency battery power distribution systems could prevent the auto closure of two of the three Safety Injection (SI) purnp breakers leaving only one SI pump available. Failure to meet the single failure criteria placed the unit into an LCO Action Statement that requires shutdown. The licensee discovered this FEBRUARY 5, 1988 ENCLOSURE B

previously unanalyzed condition during a review of the emergency power distribution system,s including the DC control power system, as a followup to an NRR request. NRR and Region II are following the licensee's resolution to this condition.

3. Shoreham On February 1,1988, the ASLB which deals with the February 13, 1986 exercise of the offsite emergency plan issued a Partial Initial Decision (PID). This PID addresses the contentions related to the licensee's performance during the exercise.

The Licensing Board found that fundamental flaws were demonstrated by the February 13, 1986 exercise of the offsite emergency plan for Shoreham.

In the PID, the Licensing Board cites threc specific areas that were fundamentally flawed. The three areas are:

(1) Communications within the EOC, among field workers, at the ENC and in the EBS messages.

(2) Traffic Control Post not staffed until well after traffic congestion would have occurred, and (3) Trainino proorans in the areas of communications, function of Traffic Guides and Bus Drivers, and promptness of field personnel.

A separate opinion was provided by Judge Frye, the Board's chairman.

Judge Frye took exception, in part, to the Board's communication and training findings. j i

0FFICE OF NUCLEAR MATERIAL SAFETY AND SAFEGUARDS Items of Interest Week Ending February 5, 1988 LOW-LEVEL WASTE MANAGEMENT Uranium Recovery Meetings With DOE On February 10 and 11, 1988, NRC staff will meet with representatives of DOE's Albuquerque Operations Office to discuss the Title I program. Areas of discussion will include the NRC review and concurrence process, DOE's QA/QC program and DOE's approach for addressing EPA's recently published draft groundwater standards.

Low-Level Waste Management Meeting with DOE On February 1,1988, Malcolm R. Knapp met with Willis Bixby to discuss NRC/ DOE interactions on the West Valley Demonstration Projtt. Topics included the Cement Solidification Process, Decontamination and Decomissioning criteria, and the concentration of TRU in West Valley low-level waste.

INDUSTRIAL AND MEDICAL NUCLEAR SAFETY Nuclear Fuel Services, Inc. (NFS) Erwin, Tennessee On February 1,1988, NRC staff (0GC, NRR, and NMSS) met with NFS corporate officials. The NFS representatives indicated that since 1979, they have made annual contributions to the escrow account which is required by a license condition for the facility decomissioning. NFS took the contribution as deductible for tax purposes. However, the 1987 tax refom act would eliminate this privilege. NFS is interested in seeking any alternative avenue which would recuce their tax burden regarding the annual contribution. Two avenues were proposed by NFS: cne is to invest the annual contribution in municipal bonds, and the other is to request their customer (D0E) to undertake the obligation for decommissioning. The NRC representatives indicated that the NRC's position on decomissioning is to ensure for health and safety purposes that the licensee has the ability financially to decontaminate the facility at the end of the plant life. The NRC staff will evaluate the licensee's proposal on the basis  !

' 2 Dry Spent Fuel Storage _

On February 1,1988, NMSS staff net with FW Energy Applications, Inc., (FWEA) and General Electric Company of the United Kingdom (GEC) regarding the review of FWEA's topical report for its Modular Vault Dry Store (MVDS) design. Much of the meeting centered on site-specific interfaces and final design inforrration requirements for a site-specific license submittal. FWEA and GEC are in serious discussions with a utility (which they decline to identify publicly) regarding the MVDS.

SAFEGUARDS Domestic Licensing of A1 Chem!E Gas Centrifuge Enrichment A meeting with All Chemical Isotope Enrichment, Inc. (AlChemIE) has been  ;

scheduled for Tuesday, February 9, in the White Flint Building. The topics to be discussed are related to staff review of AIChemIE's license applications  ;

for enrichnent of non-nuclear materials using DOE's gas centrifuge machines at Oak Ridge, TN. The AlchemIE application is unique in that it involves a '

non-nuclear use of sensitive nuclear equipment and technology. Safeguards l licensing staff will participate in the meeting to discuss safeguards and nonproliferation concerns and measures to protect against unauthorized uranium enrichment.

OFFICE OF NUCLEAR REGULATORY RESEARCH Items of Interest Week Ending February 5, 1988 REGULATORY ANALYSIS Meeting on IPE and A-45 Dr. M. Modarress, the University of Maryland, briefed Dr. B. Sheron, W. Minners, K. Kniel, F. Coffman, G. Mazetis and F. Eltawila on his review for RHFB concerning the adequacy of the IDCOR IPEMs to prescribe ways to reduce the risks covered by USI A-45 in addition to satisfying the Comissirn's Severe Accident Policy.

Dr. Modarress concluded that both the IDCOR IPEMs and the limited scope PRA used by Sfil for the A-45 studies would identify similar leading decay heat renoval (DHR) core damage accident sequences. Use of the IDCOR IPEMs would require additional utility work to study the effectiveness of any proposed fixes. The staff's draft SER already identifies the several enhancerents needed to assure that utilities which use the IDCOR nethods would discover DHR weaknesses.

PREVENT CORE DAMAGE Multi-Loop Integral System Test (MIST) Procram Management Group (FMG) Meeting, January 26-27, 1988 The MIST facility is designed to simulate thermal-hydraulic aspects of tran-sients in the Babcock and Wilcox (B&W) reactors of the lower-loop design.

The data is used in assessing the capability of NRC.and industry thermal-hydraulic best-estinate providing the Small Break codes in Loss-of-Coolant predicting Accident (B&Wdata SBLOCA) transients base end necessary to satisfy NUREG-0737 (Clarificatien of TMI Action Plan Requirements). This program is jointly sponsored by the hRC, the B&W Owners Group (B&WOG) and the Electric Power Research Institute (EPRI).

The PMG meeting started with the presentation on the overall Status of the project. Eight tests were performed under Phase IV of this program. This brings the total number to 58 tests conducted to date. B&W presented the preliminary results from four of the Phase IV tests. The remainder of the Phase IV tests are under review by B&W, Meanwhile, the facility is in a cold shutdown condition. B&W also gave an overview of the technical conclusions from the 6 MIST tests tests that investigated the affect of reactor pump operation on transient progression. In addition, B&W presented a sumary of the technical observations from test that have been analyzed.

The RELAP5/ MOD 2 analyses of a MIST tests showed reasonable agreement between post-test predictions and data, but also identified infomation needed to give better predictions. The TPAC-PF1/M001 post-test analyses perfomed by Los Alanos FEBRUARY 5, 1988 ENCLOSURE D

2 concluded that the discrepancies between the prediction and the data could be attributed to the adequacy of (a) the knowledge of facility operation, (b) the input model, and (c) the models and correlations in the code. The meeting concluded with a review of the financial status of the project.

Currently, the project is on schedule within budget. The remaining work is focused on evaluating the adequacy of the Phase IV tests in meeting their obiectives; conducting repeat test (s) if necessary; and finished various required documentation such as data analyses, post-test code analyses, summary report, etc.

Termination of Certain Themal-Hydraulic Research Budget reductions have forced tha termination of certain themal-hydraulic research activities. The TRAC-BWR code is used to model the response of BWRs to transients and loss-of-coolant accidents. This code will no longer be improved or maintained. The TRAC-BWR code was made available through bilateral agreements to Japan, F.R. Germany, Mexico, Spain and Switzerland.

These countries apply the code to independently detemine the adequacy of plant design to safely respond to transients and LOCAs, evaluate emergency operator's procedures, assist in resolution of issues, and evaluate operating transients. RES will withdraw from its international comitments for TRAC-BWR.

1 The RAMONA code is used to model the response of BWRs to reactivity transients. This code will no longer be maintained. Taken together with the ternination of TRAC-BWR, there is no assurance that analytical capability will exist for PWR transient response.

RES conducted the MB-2 progran in conjunction with EPRI and Westinghouse to provide infomation on U-tube stean generator heat transfer and transient ,

response. The experimental program is complete. The data evaluation is j being terminated so that while data was produced, it will not be utilized.  !

Disposition of GSI 122.2 "Initiatino Feed and Bleed" i l

Week Ending February 5, 1988 Incident Respense Branch On February 3, the Director, Deputy Director, AE00 and staff members met with the Deputy Assistant Secretary for Energy Emergencies and the Director of the Office of 011 and Gas, Department of Energy (00E), to discuss NRC's involvement in the Continuity of Government program and to establish a working relationship for implementing a mutually satisfactory program. General agreement was reached on the mannar in which NRC would support DOE in its overall responsibilities to assess the U.S. energy situation and take appropriate actions in the national interest. In addition, it was agreed that DOE would:

(1) keep NRC informed of relevant issues that may require NRC involvement in the national emergency preparedness program being developed by the Federal Emergency Management Agency (FEMA), and (2) provide NRC with guidance for developing and implementing that portion of the NRC national emergency preparedness program in which NRC will provide support to DOE.

- On January 27, the Director, AEOD and Incident Response Branch (IRB) staff members conducted a tour of the Operations Center and presented a briefing on the NRC and AE0D response program for Dr. Pilsoon Han, Preside:1t of the Korea Advanced Energy research Institute and his associates. The Korean representatives showed interest in how information would be passed between NRC and Korea in the event Korea was to request technical assistance under the NRC/ KOREA Cooperative Agreement.

On February 2, an IRB staff member net with DOE representatives to assist them in developing an emergency response program patterned after NRC's incident response program. The discussion covered issues related to training, technical procedures, and physical facilities.

On Friday, February 5,1988, the Director, D0A and IRB staff met with EPA, FEMA, DOE and 00J officials to resolve a concern about how the Federal Radio-logical Emergency Response Plan (FRERP) will be accommodated in the FEMA Plan for Federal Response to a Catastrophic Earthquake. (.prrently, a Federal response under the FRERP would come under the Hazardous Material group lead by EPA. The NRC staff position is that a FRERP response should not be coordinated by EPA since this is not how a FRERP response is normally handled. At this meeting, there was general agreement that a FRERP response should be handled independently even though some concerns were expressed about competing rescurces and FEMA's overall management of the response. To bring this longstanding issue to resolution, NRC agreed to develop a draft annex which would cover an independent radiological response group and then circulate the draft for review and coment.

Diaanostic Evaluation and Incident Investigation Branch On February 4, two members from the Diagnostic Evaluation and Incident Investigation Branch (DEIIB) met with Region II personnel to obtain information i needed to evaluate the best tining and scope for the Turkey Point diagnostic 1

evaluation. The meeting focused on Turkey Point's past and current inprovement initiatives and the schedule for completing each initiative.

On February 4, a conference call was held among Duke Power, AE0D/DEIIB, and Region II to discuss the planned short-tem corrective actions to be taken by Duke as a result of concerns raised during the recent McCuire diagnostic evaluation in the area of check-valve testing. Based upon the preliminary results of Duke's high priority design engineering study, reverse flow testing will be perfomed in the near future on additional check-valves in the diesel generator air start system and the auxiliary feedwater systen.

Reactor Operations Analysis Branch AE0D staff had met with NUMARC representatives in December 1987 to discuss POV hydraulic lockup and invite them to address the issue. On February 4, 1988 NUMARC, AE0D, RES and NRR staff met to follow up on our concerns. It was indicated that the EPRI Nuclear Maintenance Assistance Center has accepted the task of investigation of MOV hydraulic lockup due to excessive grease in the spring pack of the motor operator as part of the overall industry MOV Progran. AE00 will continue to monitor industry resolution of MOV operability issues.

The Director, AE0D and members of the staff met with Conmissioner Rogers on February 1 to brief him on the background and progress to date of the performance indicator program. The underlying philosophy of quantitative measures of effectiveness and relationship to plant safety was discussed and illustrated by several examples. Discussion of new indicators was deferred to the forthcoming Commission meeting this March.

Technical Training Center A meeting was held between senior nanagers from AE00, OP, NMSS, RES and NRR to l discuss the plans for implementation of a training and qualification program to  !

supplenent existing programs. A remorandum will be issued describing the  !

agreed upon program plan and the schedule goals for each phase.

ITEMS OF INTEREST OFFICE OF SPECIAL PROJECTS Week Ending February 5,1988 Authorization For Sequoyah Heatup At approximately 9:15 a.m., on February 4,1988, the Director of the Office of Special Projects released Sequoyah Unit 2 from the first of five pre-established NRC Hold-Points to allow plant heatup .for testing and shakedown activities. Sequoyah has been shutdown since August 1985. The shakedown heatup, which is currently scheduled to last approximately 5 to 6 weeks, will allow both the NRC and TVA to monitor operator proficiency and effectiveness of TVA organizational changes and to identify any additional equipment problems that may arise. After this heatup, TVA will either cool down Sequoyah Unit 2 to correct identified deficiencies or they may keep the Unit heated up awaiting Commission approval for restart.

An augmented NRC inspection plan which includes control room shift coverage has been put in place in accordance with the Sequoyah Restart Plan. An OSP manager is on-site to direct NRC restart coverage and daily site briefings are being provided to selected headquarters and regional staff.

PN-0SP-88-001 has been issued and will be updated appropriately.

A Notice of Violation and Proposed Inposition of Civil Penalty in the arount of

$25,000 was issued on January 29, 1988 to Duke Power Company (McGuire, Unit 1). ,

The action was based on an event in which the containment shield / divider barrier was found to be inoperable. This condition which existed for over four years and could have allowed higher than calculated containment pressure following a loss-of-coolant accident. (EN 88-05)

A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of

$6,000 was issued on January 29, 1988 to St. Louis University, St. Lcuis, Misscuri. The action was based on (1) permitting an individual to receive a whole body dose of at least 7.5 rens during the fourth calendar quarter of 1987, (2) failing to assess the radiation hazards or observe three separate warning lights which would have alerted the individuals, prior to entering the room, that the 8900 curie cobalt-60 teletherapy source was in an exposed e position, and (3) failing to report to the NRC within 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> that an over-exposure event had occurred. An Order Irposing a Civil Penalty in the amount of $500 was issued to Kermit Butcher, Elkins, West Virginia on February 2, 1988. The action was based on nultiple health physics violations which demonstrated a breakdown in the management oversight and control of the licensee's radiation safety program.

Of particular concern was the loss of licensed raterial (noisture density gauge). (EN 87-54A)

An Order Imposing a Civil Penalty in the amount of $75,000 was issued to Arkansas Power and Light Company (ANO-1) on February 4, 1988. The action was based on seven examples of breaches in vital area barriers and two examples of sleeping security guards. (EN 87-51A)

Civil Penalty Paid Northeast Utilities (Millstone, Units 1, 2, and 3) paid the proposed civil penalty in the arcunt of $25,000. The action was based on several violations of commitments made in the licensee's physical security plan, including violations relating to the failure to maintain the integrity of vital and protected area barriers and '3ilure to ensure that visitors are properly escorted while within the protected area of the plant. OFFICE OF CONSOLIDATION Items of Interest Week Ending February 5, 1988 White Flint Occupancy Alrost Complete With the NRR move to One White Flint North over the weekend of January 29 to 31, occupancy of the building neared the 90 percent mark. This was the largest scheduled move; it included approximately 650 out of a total of about 1,450 employees who will eventually be located in the building. The move went snoothly, although minor inconveniences concerning elevators and phone and electrical services were reported. The contractors responsible for these services will resolve most of these problems within a week or two.

The 17th and 18th floors, which will house the Commission and its staff, are still under construction, but the other floors are basically complete.

Pa rking Monthly permits for paid parking on the three lowest garage levels at White Flint were issued to NRC employees, effective February 1. Approximately 40 employees were assigned spaces at a satellite lot next to the White Flint Mall. Some 55 spaces in the garage are still being used for construction equipment and storage, but it is expected that these will become available some time next month.

NRC's building maintenance contractor is operating the parking facility under procedures established by the agency.

Smoking Lounges Eltron Smokatron air cleaners have been installed in the employee lounges ,

located in the "H" corridors on the 3rd through the 16th floors. These 1 ceiling-mounted air cleaners recirculate room air through a mechanical prefilter, an ionization chamber and collection cell, and an activated charcoal filter.

Second Building The Montgomery County hearing to determine whether to approve the developers' plans for the second building at the White Flint site has again been postponed, probably until later this month. 1. On February 2, the Regicnal Administretor, accompanied by the Deputy Director, Division of Reactor Safety, and a Division of Reactor Projects Branch Chief, was at the Grand Gulf Nuclear Power Station to review plant status. In the evening, the Regional Administrator presented certificates to newly licensed Grand Gulf reactor operators.

2. On February 4, the Regional Administrator; the Director, Division of l Radiation Safety and Safeguards; and other selected members of the Regional staff, met with representatives of Nuclear Fuel Services. Inc.,

in Erwin, Tennessee, to discuss the status of the licensee's Performance Improvement Program (PIP) and other pertinent issues.

3. On February 5, the Deputy Regional Administrator was at the Corporate Offices of the Florida Power and Light Company to attend an ENERCON Senior Evaluation Team Meeting on FPL's Independent Management Appraisal Program.

4. Representatives of NRR and a Region III representative were in the Region 11 Office this week to conduct an audit of Operator Licensing Activities. The Director, Division of Licensee Performance and Quality Evaluatien; conducted the exit interview on February 4

5. An EQ team inspection was conducted this week at the Duke Power Company's Catawba facility. The team consisted of Region II, headquarters, and contract personnel.

6. A special Balance-of-Plant inspection was conducted this week at the Virginia Electric and Pcwer Company's Surry Power Station. The team consisted of Region II and hRR personnel.

e ITEMS OF INTEREST REGION IV Week Ending February 5, 1988

1. Region IV conducted a team inspection at Fort Calhoun Station this week to evaluate the plant Radiation Protection Program. The team consisted of two Region IV inspectors and one representative from Headquarters (NRR).

2. A Quality Assurance tean inspection began at Waterford on February 1, 1988. This inspection is scheduled for two weeks. The inspection team is composed of a Team Leader, two NRC Headquarters personnel, two Region III inspectors, and two Region IV inspectors.

3. The Utah Bureau of Radiation Control issued a license on February 3,1988 to Envirocare of Utah, Inc., for a waste disposal site near Clive. Utah, for NORM (naturally occurring radioactive materials). This site will accept dry packaged or bulk NORM waste, at concentrations of up to 2000 picoeuries per gram, for disposal by land burial. Only NORM wastes not regulated by the NRC (and not regulated by a State under Agreenent with the NRC) may be disposed of at this site. It is expected that the early use of this site will be for the disposal of radium contaminated soils and materials.

4. Management representatives from Arkansas Power and Light Company met with Region IV management on February 4 to discuss the licensee's efforts 1 to improve their corrective action system. The meeting was requested by Region IV after an inspection of the licensee's corrective action system,

5. The Deputy Director, Division of Reactor Safety and the Chief, Operator Licensing Section attended the Regional Training Managers meeting in Irving, Texas, on February 4, 1988. The new NRC requalification progran was a major topic of discussion.

6. On February 5, the Deputy Regional Administrator and the Regional State Liaison Officer met with various state officials in Austin, Texas, and represented NRC at a ceremony in the Governor's office. Governor Clements will recognize the 25th anniversary of Texas' Agreenent State Progran and will designate February 29 - March 4 as Radiation Protection Week.

There were no requirements placed on the staff at this briefing.

Chairman Zech requested the staff to convey to the licensee that the Commission believes it appropriate for TVA to address the Commission on accomplishments implemented to improve coordination between corporate headquarters and nuclear plants. ,

In addition, the Chairman requested the staff to obtain a i commitment from TVA that the improvements will be continued.

Chairman Zech also requested the. staff to address during the next Commission briefing, TVA's effectiveness in shifting to a mode of reactor operation (e.g., disciplined control room, I attention to detail, operator proficiency, plant staff support, maintenance programs, etc.) and staff's program to oversee TVA's operational readiness and performance.

In addition, the staff should address the concerns raised by the American Nuclear Insurers of TVA's Plant Operations Review Comnittees on Safety Oversight, Commissioner Bernthal cautioned the staff to insure that plants are not permitted to deteriorate while in layup status, i
