Dear Mr. Reid:

SUBJECT: NOTICE OF VIOLATION (VERMONT YANKEE INSPECTION 94-13)

Messrs. H. Eichenholz and P. Harris conducted a safety inspection on May 15 - June 25,1994

at the Vermont Yankee Nuclear Power Station, Vernon, Vermont The preliminary results were

discussed with Mr. R. Wanczyk at the conclusion of the inspection. The inspection focused on

issues important to public health and safety, and consisted of performance observations of the

conduct of operations and maintenance, and independent evaluation of safety system status and

quality records.

As a result of our inspections we identified deficient performance of safety review activities by

your Plant Operations Review Committee. Their failure to conduct a required facility operations

review resulted in the failure to detect a potential safety hazard associated with the cross- ,

connection of the fire water and service systems used by operators to respond to the loss of all

fire water pumps at the station. Although fire water suppression capability was restored by the

cross-connection of the systems, the service water system was rendered inoperable. Also, an

alarm response procedure used in response to reactor coolant system leakage past isolation valves

in the core spray system was reviewed and approved by the Committee withaut assuring that

design features, equipment performance capabilities, and appropriate operator guidance would  ;

be available to facilitate the proper timely response for degraded equipment conditions. These j

examples of poor safety assessment performance have been identified as a violation of safety  ;

review requirements contained in your Technical Specifications. ,

respond to this letter and should follow the instructions specified in the enclosed Notice. The i

responses directed by this letter and the enclosed Notice are not subject to the clearance

procedures of the Office of Management and Budget as required by the Paperwork Reduction

Act of 1980, Public Law No. 96.511.
