One unresolved item was identified concerning discrepancies observed during

the performance of surveillance testing activities.

One portion of that

unresolved item, which is discussed in paragraph 13 of the enclosed report,

involves the observation that during performance of loss of offsite power with

safety injection testing, your staff did not recognize that loads which were

normally supplied by the 1-4 vital inverter had been transferred to the 480 V

-

shutdown panel (maintenance supply) in order to accommodate

inverter

maintenance.

Consequently, when the test was initiated, these loads were

inadvertently deenergized in an unplanned manner.

In your response to this

letter, you should also discuss the cause for this apparent coordination

- Youri attention is called to- the' commitment made by TVA during the exit

interview regarding a revision to A01-27, Control Room . Inaccessibility, to

reflect regulatory guides 1.6.8.2 and 1.6.8.

This commitment is addressed in

paragraph 3 of the enclosed report.

In accordance with Section 2.790 of the NRC's "rules of Practice," Part 2,

Title 10, Code of Federal Regulations,, a' copy of this letter, its enclosures,

and'any reply will be placed in the'NRC's Public Document' Room.

The responses directed by this. letter and its enclosure are not subject to'the

clearance procedures of the Office of Management and Budget as required by the

Paperwork Reduction Act of 1980, PL 96-511.
