NRC REGION I COMBINED INSPECTION REPORT NOS.
50-220/93-15 AND 50-410/93-15
This letter transmits the NRC Region I inspection report for the inspections conducted by
Mr. R. Bhatia of this office on July 19-23, 1993, at Niagara Mohawk, Salina Meadows
Office in Syracuse, New York, and at Nine Mile Point Units 1 and 2, Scriba, New York.
Mr. Bhatia discussed the findings of this inspection with members of your staff at the
conclusion of the inspection.
Areas examined during this inspection focused on assessing the offsite power supply sources
for Nine Mile Units 1 and 2 and your completed corrective actions to close the previous
Electrical Distribution Functional Inspection (EDSFI) outstanding items for Unit 1. The
inspection included the ongoing breaker replacement effort to further enhance the offsite
power supply sources for Unit 2, based on several concerns generated by the loss of power
events during the last two years. Within this scope, the inspection consisted of selective
examinations of design records, procedures and representative records, interviews with
personnel, and observations by the inspector.
Overall, the inspector concluded that the Nine Mile Point 1 and 2 offsite power sources are
reliable. We note that you have taken adequate steps to assure corrective and preventive
measures to enhance the reliability of the station offsite power supply sources. Further, you
are replacing the four less reliable 115 kV breakers in the Scriba substation during the 1993
fall refueling outage of Unit 2. In addition, the corrective actions for three previously
identified issues were found to be adequate and the items are closed. One other issue
reviewed pertaining to the emergency diesel generator transient loading analysis was found
partially complete; however, because of the low margin obtained in your calculation results,
you have taken
