Dear Sir:

Reforence (b) documents inspections conducted by Mr. H.

Eichenholz and Mrs. C. Carpenter during the period October 27, 1987 through January 19, 1988 at the Yankee Nuclear Power Station, Rowe, Massachusetts. The report identified an activity that was not conducted in full compliance with NRC requirements. This actavtty was identified as a Severity Level IV violation (Supplement I). A one week extension in response was obtained from our Restdent Inspector. In accordance with 10 CFR 2.001, we hereby submit the following information:

VIGLATION 10 CFR 50, Part 50.72 (b)(i)(A) requires the 1icensee to notify the NRC Operations Center via the Fmergency Notification Syutem as soon as practical and in all cases within one hour of the occurrence of the initiation of any nuclear plant shutdown required by the plant's Technical Specificat2ons (TS).

Contrary to the above, the requirenent of 10 CFR 50.72 (b)(i)(A) was not met on December 3, 1907, in that che NRC was not notified within one hour of the initiation of plant shutdown as requirerf by TS 3.0.3. Specifically, the channel 1 main steam line pressure switch on the No. 3 steam line was declared inoperable at 10:40 a.m. The TS Table 3.3.2 Action Statement 6 requires that the inoperable channel be placed in the tripped condition within one hour; if not, plant shutdown is to be initiated at 11.40 a.m. as required by TS; however, the required ENS r:otification to the NRC per 10 CFR 50.72 (b) was not made until 1:40 p.m., one hour late.

FYR 88-33 YAFC RESPONSF As identified in the inspection report, this notification failure is attributed to personnel error on the part af the Shift Supervisor to make the required determination of reportability. Immediate corrective action following the identification of the violation included the re-instruction of all Shift Supervisors and Operators on the need to make proper determinations and timely notifications on reportable events. Yhis instruction was ircorporated into the aqerator training segment which began on December 8, 1987 and was completed on January G, 1988. A Special Order h a -;

also been issued by the Operations Manager which cills further attention to late notifications and the need to comply with reporting requirements. Results achieved thus far are a heighteneo awareness and sensitivity to the reportinq requirements.

Additional training will be incorporated into the next segment of the operator training cycle which is scheduled tc begin on March 22, 198G. This training will review the response to this violation and further emphasize the notification requirements in a discussion given by the Plant Superintendent and the Operations Manager at the beginning of each class in the segment. This instruction will aiuo emphasize the need to re-assess an event periodically for changing conditions and potential r.hanges in event classification and reoartability.

Long ter m corrective attion wi11 include the development and implementation of a procedure fcr potential reportable events. A draft procedure is cur r ent l y in review and will establish a consistent r.roceus for the determination of reportable events os well as establish the policy on the timing of classifications and the reporting of the findings.
