O APPPENDIX A NOTICE OF VIOLATION Maine Yankee Atomic Power Ccmpany Docket No. 50-309 Based on the results of the NRC inspection conducted August 21-23, 1978, it appears that one of your activities was not conducted in full compli-ance with NRC regulatio. s and the conditions of your license as listed below.
This item is a deficiency.
Technical Specification 0.8 states in part: " Written procedures shall be established, implemented, and maintained covering... the applicable l
procedures recommended in Appendix A of Regulatory Guide 1.33, November 1972... ".
Regulatory Guide 1.33 lists, in part, under Section 1.
" Administrative Procedures",... " Log Entries, Record Retention and Review Procedures."
Plant procedure 0-10-3, " Maintenance of Operations Department Logs" states that for the Operations Log (now maintained by the Control Room Operators), "This log shall provide a chronological history of plant operating events, activities and/or problems..."
Contrary to the above, on August 21, 1978, it was determined that no entry was made in this log on July 31, 1978 concerning an operator error while handling fuel, which resulted in the deformation of a fuel element.
