Inspection No. 50-309/78-16 In reply to Reference (b.), the Maine Yankee Atomic Power Company hereby submits the following information:

Apparent Item of Non-Compliance:

Technical Specification 5.8 states in part: " Written procedures shall be established, implemented, and maintained covering. . . the applicable procedures recommended in Appendix A of Regulatory Guide I.33, November 1972...". Regulatory Guide I.33 lists, in part, under Section I. " Administrative Procedures",... " Log Entries, Record Retention and Reviaw Procedures." Plant procedure 0-10-3,

" Maintenance of Operations Department Logs" states that for the Operations Log (now maintained by the Control Room Operators),

"This log shall provide a chronological history of plant operating events, activities and/or problems..."

Contrary to the above, on August 21, 1978, it was detemined that no entry was made in this log on July 31, 1978, concerning an opera-tor error while handling fuel, which resulted in the deformation of a fuel element.

Although the Operations Lug contained no entries relative to the fuel handling incident associated with fuel assembly EF007H, the _

Reactor Engineering Refueling Log did contain entries relative to this incident. During refueling fuel handling operations, special fuel han?1ing crews and supervisors are established to perform and N/N hY .-

These fuel handling crews are established to provide close monitoring of these operations and to relieve the Plant Shift Superintendent of these additional respon11bilities. These personnel maintain their own log of events relatise to fuel handling operations. This log is considered to be a " supplement" to the Operations Log providing additional infor-mation specifically related to the fuel handling operations. This log was available and was reviewed by the inspector during his inspection at the Plant.

Maine Yankee contends that the fuel handling incident was, in fact, documented but that supplemental information relating to subsequent activities was not recorded in either of the aforementioned logs.

As a result, and to prevent a reoccurrence of this type of incident, the operators have been reminded of the importance of maintaining an accurate and complete chronological history of plant operating events, activities and/or problems.
