Based on the results of the NRC inspection conducted on August 21-24, 1978, it appears that certain of your activities were not conducted in full com-pliance with NRC requirement.

A.
Section 6.8.1 of the Technical Specification states that written pro-cedures and administrative policies shall be established, implemented and maintained that meet or exceed the requirements and recommendations of Section 5.1 and 5.3 of ANSI 18.7-1972 and Appendix A of Regulatory Guide 1.33, November 1972.
Sect ion C.1 o f IINP-821 (Quality Control Work Inspection) states that work may not proceed beyond mandatory inspection liold points without QC inspection.
l Contrary to the above, on March 28, 1978, during the performance <.
l Maintenance Request 78-635 (CRD Replacement), work proceeded beyond I
the first four mandatory inspection hold points without yC inspet tion.

Section F.4.6 of ifNP-820 states that a master records requirements matrix shall be prepared and utilized to determine and audtt the receipt and update the int ernally generated records and dm ume:ta-tion.
Contrary to the above on August 24, 1978 a master records requiremment matrix had not been prepare 1.
This is a detitiency.
