SUBJECT: NRC INTEGRATED INSPECTION REPORT 50-354/97-02 i

NOTICE OF VIOLATION l

.

Dear Mr. Eliason:

On April 28,1997, the NRC completed an inspection at your Hope Creek reactor facility.

The enclosed report presents the results of that inspection. j

j During the 6-week period covered by this inspection period, your conduct of activities at j

the Hope Creek f acility was generally characterized by safety-conscious operations, sound i

engineering and maintenance practices, and careful radiological work controls.  ;

During this period, visiting inspectors: conducted routine inspections of your site physical

protection program; provided oversight of your own service water system operational

performance inspection; and, provided extended control room observation inspections.

! The details of the physical protection inspection are contained in NRC Inspection Report

50 272/97-07;50-311/97-07, and the conclusions of that inspection are included in this

! report. The inspection consisted of selective reviews of procedures and records,

inspectors' observations, and interviews with security personnel. The inspectors

determined that you are implementing a security program that protects public health and

, safety. The details of the other region-based inspections are contained in this report.

4 We are concerned, however, about a violation of NRC requirements that was identified

during this inspection. in this instance, NRC inspectors identified that equipment operators

failed to adhere to emergency diesel generator system operating procedures, in that they

failed to ensure the proper lineup of the diesel cylinder petcock valves prior to starting the

diesel. While this condition was immediately recognized by the attendant operators and

prompt action taken to prevent the condition from causing any damage to the equipment in

the room, we are concerned that the event should have been avoided. We previously

noted a similar condition that had occurred in 1996, reference Inspection Report 50-

354/96-80, in which your operators identified the petcock valves open following a

maintenance activity, and prevented a start of the diesel generator at that time until the

valves were closed. While there are noted differences in the causes of these two events,

we were concerned that the lessons learned from the 1996 event had not been fully

effective.
