Appendix'A NOTICE OF VIOLATION

Cincinnati Gas and Electric Docket No. 50-358 Company Based on the results of an NRC inspection conducted on August 2-4, 1978, it appears that certain of your activities were in noncompliance with NRC requirements as noted below. Items A and B are infractions, and Item C is a deficiency.
A.
10 CFR 50, Appendix B, Criterion VII states, in part:
"Docu-mentary evidence that material and equipment conform to the pro-curement requirements shall be available at the nuclear power plant site prior to installation or use of such material and equipment."
i Wm Zimmer Nuclear Power Station Unit 1 FSAR Chapter 17, Paragraph 17.1.7.6 states, in part:
" Documentary evidence of an item's con-formance to procurement requirements is required to be available at the construction site prior to installation or use of that item."
Contrary to the above:
(1) No documented evidence could be provided to assure that the Reactor Building Closed Cooling Water Fump Motors and the Service Water Pump Motors were capable of starting at the applied voltage values or capable of developing the break-down torque values, as stated in the Wm H. Zimmer Nuclear Power Station Unit 1 FSAR Question 221.94 (8.3.1.1),
answers 1 and 2.
The motors were released for installation.
(2) No documented evidence could be provided to assure that the 53 l
differential pressure switches, supplied on purchase order No.
2475 and material receipt No. 38839, met the specified require-ments of IEEE-323-1971 and site environmental conditions (i.e.,
temperatute, pressure, relative humidity, and radiation.
These requirements were specified in Specification No. H-2866, Division 3.
The instruments were released for installation.
B.
10 CFR 50, Appendix B, Criterion III states, in part: " Measures shall.
. assure that applicable regulatory requirements and design basis.... are correctly translated into specifications, drawings, procedures and instructions."
Wm Zimmer Nuclear Power Station Unit 1 Quality Assurance Manual Section 3, Paragraph 3.3.1 states, in part: " Specifications are prepared b the designers, which clearly translate the design principles into acceptable structures, systems, and components."

Appendix A Contrary to the above, Sargent & Lundy failed to translate into the applicable site electrical specification, H-2173 Supp. 5, 5-25-78 (Paragraph dl.), the clearance requirements between conduits which contain redundant cables, as specified in Wm Zimmer Nuclear Power Station Unit 1 FSAR Chapter 8, Paragraph 8.3.1.11.2.4.
C.
10 CFR 50, Appendix B, Criterion V, states in part:
" Activities affecting quality shall be precribed by documented instructions, procedures.
Wm Zimmer Nuclear Power Station Unit 1 FSAR, Chapter 17, Para-graph 17.1.5.1 states in part:
" Activities affecting the quality of the facility are accomplished in accordance with written in-structions, procedures, or drawings which prescribe acceptable methods for carrying out the activities...."
Wm Zimmer Nuclear Power Station Unit 1 FSAR Appendix C, (dated February 1976) commits to comply with Regulatory Guide 1.58 Revision 0, and therefore, ANSI N45.2. 6-1973, which states in part in Section 1.3:
"It is the responsibility of the organi-zation performing these activities to specify the detailed methods and procedures unless they are specified in the contract documents."
Contrary to the above, written 1nstructions or procedures were not provided to assure full compliance with ANSI N45.2.6-1973 for at least two organizations which have been performing safety related inspection or test activities at Wm Zimmer Nuclear Power Station Unit 1.
For example, (1) The CG&E Electrical Operating and Test Department (E0TD) has not prescribed in written instructions or procedures how the test department assures compliance with ANSI N45.2.6-1973 Sections 2.2.4(5) and 3. 2.1.
(2) The contractor (Waldinger/ Young and Bertke) responsible for installation and inspection of safety-related HVAC supports and systems failed to prescribe in written instructions and procedures how they assure compliance with ANSI N45.2.6-1973 Sections 2, 3, 4 and 5.
