Reacter vessel level decreased rapadtv to the scram setpeant and contansed to drop to the NPCS and Reacter Core Isolatten Cooling (RCIC) instaatsen setpeants 1103.3 anchest. The lowest vessel level reached was 100 a nc he s. Vessel level was returned to the i normal range. and both the NPCS and RCIC p q s were secured. A feed pump was subsequently restarted to meantaan level. Ine DNU$UAL EVENT was declared at 5:45 p.m. and termanated at 6.02 p.m.. One fare to the south of the sale at an abandoned lumber ware-house presented me concern. The other fire, appresametely one male northwest of the plant, savolved a chemacal dastributaon warehouse, which stored tenac chemacals and, as a general licensee, was reported to have stored small quantattes of radaeactave materaals New Hampshire as an " A g reeme n t State" and the State Radiatsen Health Program manager had bee 9 notafaed about the fare. The State Polsce were notatted, came to the sale, and arrested the andswadual. The vehscle was impounded. The indswadual anwolved worked f or a janatorial cont ractor. The contractor has been used prewsously by the licensee, however, the contractor's employees are always under escort when they enter the protected area. Latensee/Facality Notiftcation/ Subject Millstone Unit 1 3/14 RI PC DN 50-245 Reactor Scram Event Whale at full power, at 5 18 am, 3/12, the "A" Reactor Feed Pump (RFP) trapped. An operator ammedaately started the standby RFP. but was unable to prevent the scram tatensee review showed that, due to a dart contact an t he f eed Wat er Control System (fWCS). the Feedwater Regulatang Va wes IF RVsl had shut when RFP-A trapped. The contact was cleaned and the FWCS then tested satisfacternly. The lacensee was unable to determane the cause of t he RF P-A t r ap. Startup began at 12.23 am. 3/13 cratscalaty was achaeved at 5'Is am. The startup was halted about 7.00 am because the !?censee was unable to comply with has amplement ang proc edure for IS 3.6 8.3, "Primary System Boundary. Core Cratacalaty.* Thas TS requares several vessel and coolant temperatures to be abowt the lamais gaven an TS Fagure 3 6.3. An August 1987 15 amendment added the "bottom head draan" to these parameters. The Am p ated Inspectaen Team sent to Cat awba March le is stall en site. Usat 1 is currently at 14% power af ter receiving concurrence from NRC to restart. Stagnant Unat 1 service water lines (with the exception of the spent fuel pool line) that could have been affected by clam $ were flushed and spot w asual chec ks er radtegraphs conducted praer to i restart. Use of the spent f uel pool line is under a JCO because t here are components an the lane that could be flew rest ric t ed by Class. ne clam problems were detected on Unit I during the inspectaens. Unnt 2 as sta!! an Mode 4. The !!censeee has checked the pressure swatches en the assaltary feedwater syriem (AFW) which Anitaate the swatchever to seewace water and found them to be functionang properly. The licensee belsewes than the low AfW pump sec t aen pressure, whac h caused the over to service water, was caused by improperly f unct ionan9 check walves from eermal supply sources. The lacensee is investagatang this suspected problem. Over the weekend the 1.censee found mere class se the two A6 W f low cont rol valves (FCV) whach had been clogged eariner. Today the latensee's plans saclude more flushang of the affected AFW llanes, anspectaen of the FCV's, anspection et specafac AFW check valves and a dynamic test of the AfW system. The lacensee has c omple t ed flushang of other dead legs an the service water system, no addational problems were reported. Unst 2 as still covered by the Confarmatsen of Actnen letter assued last week and the licensee must get NRC concurrence praer to restart. In response to the alarm and to the DRPI andicatten, t he nuclear st at ion operator antt iat ed a manual reac ter sc ram and, as directed by the shaft engaaeer. Anatsated a 100 ppe heration. Subsequent investigation by the licensee determined that the DRPI andication was due to a circoat malfunctaen as traan A et the DRPI system; control red D4 had not mover when the system anescated a red height of 18 steps. On t he mornang of March 12 1988, at about 10:30 a.m. (CST). the licensee was rewaewang requesta to the safety valve vender (Crosby) for lift setpeant testang for the Unat 1 safety valves. These requests, whach were being prepared as an prewaeus outages, requested that the valves be laft tested and that "as left* 1 aft values would be *r_2% of the setpeants specafaed an Table 4.F-1 of the Zaen 15. It was dascovered that Zaen 15 requare that the tolerance for laft setpoints is er_1%. A review of t he Uns t 2 safety valve test data from the prewaeus eutage revealed that three safety walves had as left" latt setpeants outsade the er_11 tolerance. Two of the three valves are located en the A steam lane, and t he ot her valve is en the D steeg lane. The valves were 0.11. 0.48. and 0.03% out of tolerance respectively. The incensee declared the three safety walwes inoperable at 1:37 p m. (C$T) and anatsated the Unal 2 power reductaen from 991 power at 1 45 p m, (CST l. Reacter power reached 65% at 2.30 p.m. (CST). and the NIS hahh flem reaCler trap setpeants were rescaled to F4Z by 3:45 p.m. (CST). 15 J.7.1.; requares that the MIS trap setpoants be reduced withan 4 h5ers. THE BRE ACH WAS CAUSED BY THE REMOVAL OF A SECTION OF VENTILATIDN DUCT BY MAINTENANCE PERSONNEL WHICH CONTATNED A TURNING VANE BARRIER INSIDE CONTROLLED ACCESS IN THE UNIT 2 AURILIARY BUILDING ON THE 404 FOOT ELEVATION THIS COULD HAVE AELOWED ACCESS TO MOST VITAL EQUIPMENT AREAS IN BOTH UNITS BY ENTERING THROUGH DUCT F ROM THE AUXILIARY BUILDING ROOF, A GUARD WAS POSTED AND A SEARCH OF THE A RE A F OUND NO PR OBLE MS. ACCESS TO THE DUCT WOULD HAVE REQUIRED REMOVAL OF A SCRE EN AT THE ENTRY POINT T3 TH[ DUCT. THE SCR((N WAS FOUND INTACT. AT THE TIME UNIT 1 WAS AT 50 PERCENT PDWER AND UNIT 2 WAS DEFUELED IN MODE 6.
