Gentlemen:
Thank you for your letter (serial P-86046) of January 17, 1986, in response t1
our letter and the attached Notice of Violation dated December 18, 1985. As a
result of our review, we find that additional information, as discussed with
your Mr. Fuller and Mr. Evans (and other members of your staff at an exit
interview on February 13,1986) is needed.
Specifically, your revised Procedure
50P S2-04, Section 2.2, "Startup Check List for Automatic Operation," instructs
the operators to confirm the availability of service water, to check the lube
oil level, and to verify that the engine water jacket heaters are operating.
Your corrective action still does not delineate which valves, plugs or similar
components to check in verifying correct system lineup.
Consequently, it also
does not provide a means for the operator to document that all applicable
components were checked as correctly aligned. The procedure still relies on
the operator's familiarity with the system.
In addition, the corrective action
does not address independent verification.
Please provide the supplemental
information within 30 days of the date of this letter.
