1 250-85-01 SH VALVEX Following a review of radiographs three auxillary feedwater steam supply stop check valves were declared out of service due to broken guide studs. The studs had failed due to high cycle fatigue.

The valves were repaired and tested

250-85-04 ZZ ZZZZZZ The unit was shutdown in accordance with Technical Specifications due to the identification of debris in two emergency containment coolers. As

a. conservative measure the coolers were administrative 1y declered inoperable.

The debris was apparently placed in the coolers during a unit outage.

During the start up from this outage a main steam isolation valve had to be repaired which delayed the return.

Following the repairs, the unit came on line and resumed full power operation.

The leak was repaired and the unit was returned to the grid.

3 LER 389-86-1 RC FUELXX During power ascent, the unit tripped on Local Power Density due to Axial Shape Index (ASI) oscillations. The core was near the end of core life and is experiencing ASI oscillations.

The oscillations subsided and the unit was returned to power.
