MONTICELLO NRC INSPECTION REPORT NO. 50-263/97006(DRP)
Dear Mr. Wadley:
On May 27,1997, the NRC completed an inspection at your Monticello reactor facility.
The enclosed report presents the results of that inspection.
During the 6-week period covered by this inspection period, your conduct of activities at
the Monticello facility was generally characterized by safety-conscious operations, sound
engineering and maintenance practices, and careful radiological work controls. Your
decision to shut down the reactor on May 9 to replace the torus suction strainers
demonstrated a conservative operations' philosophy,
in the previous inspection period, we discussed a loss of voltage to e.n essential electrical
bus and subsequent start of the emergency diesel generators. The loss of voltage
occurred because adequate procedure guidance was not provided to the electricians for
verifying continuity across relay contacts. We completed our assessment of this event
and determined that it met the criteria for a licensee-identified, non-cited violation. During
the current inspection period, we identified a violation of NRC requirements. Specifically,
inadequate turnovers and panel attentiveness resulted in operations personnel being
unaware that the #13 emergency service water pump had been operating for 46 hours5.324074e-4 days <br />0.0128 hours <br />7.60582e-5 weeks <br />1.7503e-5 months <br />.
This event, in combination with operations' related issues, culminated in a management
meeting between NRC management and you and your management staff on May 27,
1997. As stated at that meeting, we request you continue to place appropriate
management attention on addressing these operations performance issues.
This violation is cited in the enclosed Notice of Violation, and the circumstances
surrounding the violation are described in detailin the enclosed report. Please note that
you are required to respond to this letter and should follow the instructions specified in the
enclosed Notice when preparing your response. The NRC will use your response, in part,
to determine whether further enforcement action is necessary to ensure compliance with
regulatory requirements.
