BACKGROUND A Nuclear Safety Review Staff (NSRS) investigation was conducted to determine the validity of an expressed employee concern received by the NSRS. The concern of record, identified as SQN-85-001-02, stated:

There is fiberglass insulation at the bottom of each steam line next to the steam generator. CI has been

told that there should be no insulation other than mirror inside containment. December 1985 timeframe.

II.

SCOPE A.

The scope of this investigation, as defined by the employee concern, is that fiberglass insulation was being used on steam lines inside containment.

B.

NSRS and Plant Metallurgical Engineering personnel inspected the

. steam lines on the bottom of the steam generators of unit 1.

Findings

1.

Regulatory Guide 1.36 prohibits all use of materials containing chlorides coming in contact with austenitic steel pipe. Mirror insulation contains no chlorides.

2.

When the NSRS and Plant Metallurgical Engineering personnel inspected the steam lines inside containment unit 1, only metallic mirror insulation was noted.

Discussions with plant personnel revealed that an extensive qualification program is underway to ensure that all nonmetallic thermal insulation meets U.S. Nuclear Regulatory Guide 1.36, dated February 23, 1973, " Nonmetallic.Theruml Insulation for Austenitic Stainless steel." This is being accomplished with sample testing of nonmetallic insulation in use inside containment. A full walkdown of all piping systems within containment in units 1 and 2 is scheduled to ensure that all nonmetallic thermal insulation installed in containment meets Regulatory Guide 1.36.

NSRS has done a full investigation on nonmetallic thermal insulation on austenitic stainless steel systems (see NSRS Investigation Report No. I-85-106-SQN) which covers the use of fiberglass insulation and other nonmetallic types of insulation on stainless steel piping such as the pipes in question.

IV.

CONCLUSIONS AND RECOMMENDATIONS A.

Conclusions 1.

The employee concern that fiberglass insulation is at the bottom of each steam line next to the steam generator was not subatantiated. Mirror insulation was noted at this location.

2.

The concern that no insulation other than mirror insulation should be inside containment is covered in NSRS Investigation Report No. I-85-106-SQU.

Four recommendations on this report are being implemented by Sequoyah Nuclear Plant.

Employee Concern IIN-86-259-X13 Concern: CI stated that there are numerous foreign objects embedded in the concrete through out the plant.

i.e. nails, wire lights, bolts, nuts. No specific location was provided. Construction concern. C1 has no additional information.

Conclusions:

The concern was substantiated as embedment of foreign objects in concrete pours has occurred. However, the effecto,have been mitigated by the use of the NCR process and concrete repair procedures. It was evident that controls such as those in G-2 were in place to inspect cleanness prior to concrete placement. In addition, inspection personnel appeared to' be aware of cleanliness requirements; and concrete placement was to take place only in the presence of authorized inspectors. It was noted by NSRS that WBN-QCP-2.02 did not directly require an inspection for cleanness prior to concrete placement. However, this did not appear to be the cause of foreign material embedment. This problem appeared to be caused by the difficulty in actually performing the inspection.

NUCLEAR SAFETY REVIEW STAFF REPORT No. I-85-472-NPS NSRS Report No. I-8 5-4 72-NPS A Nuclear Safety Review Staff (NSRS) investigation was conducted to determine the validity of two expressed employee concerns as received by Quality Technology Company (QTC)/ Employee Response Team (ERT). The concerns of record, as summarized on the Employee Concern 4ssignment Request Forns from QTC and identified as XX-85-062-002 and IN-86-108-002, stated:

Concern: The current TVA system of using a master card file of drawings (this system in place at Sequoyah) is inadequate and should not be implemented (if not already) at Watts Bar, Browns Ferry, and Bellefonte. This system allows for instant disarray, destruction, or loss of reference cards if a drawer is dropped or the system sabo-taged. The data from the cards should be input to the computer and available to all applicable offices within TVA.

Conclusions 1.

The employee concern is substantiated not on the specifics of the concern but its substance. The basis for this conclusion follows:

a.

The " current TVA system" for configuration control is intended to be the DMS not the master card index system as alleged.

However, upper-tier instructions do not preclude the use of the master card file and, therefore, though SQN DCC personnel are complying with the updating of D}$ for SQN CC drawings in accordance with upper-tier (reference 1 and 2) requirements, they are not meeting the literal intent of the requirements to use DMS exclusively, since SQN DCC personnel continue to maintain and utilize the caster index.

The SQN master index system is not a reliable CC drawing control system since it is not controlled nor is it an approved QA program method.

c.

The master index is inadequate for TVA use because it is a localized system which does not provide accessibility to other TVA organizations having a need to obtain the proper as-constructed status of SQN equipment.

~ 2.

Accordingly, P&E (Nuclear) decided to replace all the systems such as the master index with the DMS as the single CC drawing management system for P&E (Nuclear). Browns Ferry, Watts Bar, and Bellefonte do not use a master index system for CC drawing control but do use DMS.

Recommendations 1.

I-85-472'NPS-01, Configuration Drawing Control Requirements The upper-tier requirements contained in references 1 and 2 do not preclude the continued use of master index systems for construction status and other information retrieval for CC drawings. NSRS recommends:

a.

That references 1 and 2 be revised to require DMS utilization exclusively by site DCC organizations for CC drawing information retrieval, and i

b.

that the master index be placed under the QA program as a backup system, if desired, or until total conversion to DMS occurs.

(P2) 2.

I-85-472-NPS-02, SQN Imolementation of DMS SQN DCC has not implemented the D>G as the single CC drawing manage-ment system. NSRS recommends that SQN revise reference 19 to require exclusive utilization of DMS for CC drawing information retrieval.

This action should establish the D>$ as the single CC drawing manage-ment system and discontinue DCC utilization of the master index.

Sequovah Nuclear Plant (SQN) Response to I-85-472-NFS-01 The TVA Configuration Control Task Force is continuing their ef forts to develop progra=s for the use of the Drawing >bnagement System (DMS).

The areas still being developed include auditing of the system, methods to ensure data is correctly entered, number of terminals required, etc.

Until the task force has developed all their programs to ensure the D}G can be completely i=plemented and all other systems deleted, a revision to references 1 and 2, identified in the NSRS report, will not be requested.

The master card index in the SQN Drawing Control Unit (DCU) is used for information by site personnel and does not require being placed under a quality assurance program. Several constraints within the drawing distri-bution systen will prevent the issuing or using o*f out-ofadate drawings even if information contained on the cards in the master card index is incorrect or missing. Workplans contain a specified drawing revision level to be used, so the cards have no impact on the workplan. Also, if an individual needs a design drawing, he must know the revision level needed or he will be issued the latest revision in the DCU drawing tank.

When "As-Constructed" (AC) drawings are requested, the individual is issued the latest AC revision contained in the DCU drawing tanks even if the card index shows the wrong revision.

In addition, all the information is con-tained in the DMS, and if cards are lost, the information can be repro-duced. The master card index is used by site personnel as a quick reference and for information only, and the information contained on the cards will not result in incorrect drawings being issued or used. Also, the DFG computer printout is used by the DCU personnel to perform audits of the remote distribution points to ensure the latest drawing revisions are available for use..,,

" Receipt, Filing, and Distribution of Drawings," will not be. revised to require exclusive use of the DMS until the Configuration Control Task Force has all the programs in place to facilitate implementation of the DMS.

In addition, the SQN DCU has requested additional computer '

terminals to support increased use of the DMS. At the present time, only two computer terminals are available for use with the DMS in the DCU.

Cable Replace the flexible conduits previously described, and evaluate and replace as necessary the associated cable.

Determine and eliminate the root cause of the damage.

These components are transferred to Watts Bar ONP, therefore,

According to Mr. Dan Thompson, NSB, the maintenance request A-477393 will

, require inspection and replacement as necessary.

The estimated completion date is March 14, 1986.

NOTE: Heat tracing circuits are not under the quality assurance program.

The root cause appears to be the grounding of welding leads through the conduit. Therefore, an agenda will be written for the March 31, 1986, safety meetings covering the proper methods of grounding while welding. All crafts will be made aware of the electrical hazards and destruction that can be caused by improper grounding techniques.

The potential hazard created by welding to close to conduits which may contain energized cables, will also be pointed out in this meeting.
