Gentlemen:

This refers to the inspection conducted by Mr. J. G. Luehman of this office ,

during the period February 1-28, 1986, of activities authorized by NRC

Operating License NPF-38 for Waterford Steam Electric Station, Unit 3, and to

the discussions of our findings with Mr. R. P. Barkhurst and other members of

your staff at the conclusion of the inspection.

Areas examined during the inspection included plant status, routine inspection,

monthly maintenance, monthly surveillance, Licensee Event Report (LER) followup,

followup of previous inspection findings, followup of previously identified

items, allegation followup, and Inspection and Enforcement Circulars and Bulletins.

Within these areas, the inspection consisted of selective examination of procedures

and representative records, interviews with personnel, and observations by the

inspector. These findings are documented in the enclosed inspection report.

During this inspection, it was found that certain of your activities were in

violation of NRC requirements. Consequently, you are required to respond to

this violation, in writing, in accordance with the provisions of Section 2.201

of the NRC's " Rules of Practice," Part 2, Title 10, Code of Federal Regulations.

Your response should be based on the specifics contained in the Notice of

Violation enclosed with this letter; and should include the total radiological

impact of the two inadvertent releases as well as why plant instrumentation and

control personnel.were allowed to conduct testing a second time without further

guidance or procedures.

The response directed by this letter and the accompanying Notice is not subject

to the clearance procedures of the Office of Management and Budget as required

by the Paperwork Reduction Action of 1980, PL 96-511.
