APPENDIX A NOTICE OF VIOLATION
-Houston Lighting & Power Company Docket Nos.: 50-498/499 South Texas Project During an-NRC inspection conducted' January 6 - February 14, 1986, a violation of NRC requirements was identified. The violation involved the lack of qualified procedures.
In'accordance with the " General Statement of Policy and Procedure for NRC Enforcement Action," 10 CFR Part 2 Appendix'C (1985), the violation is listed below:
Failure to ' provide written instructions or procedures 10 CFR 50, Appendix B, Criterion X requires that a program for inspection of activities shall be established and executed...to verify confonnances with documented instructions, procedures, and drawings for accomplishing the activity.. It is further required that if mandatory inspection hold points which require witnessing or inspecting...beyond which work shall not proceed are required, these shall be indicated in appropriate documents. Criterion XVI requires that conditions adverse to quality are to be promptly identified, documented, and corrected.
Revision 12 of the licensee's " Quality Assurance Program Description,"
(QAPD), Section 15.0, " Nonconforming Materials, Parts or Components,"
requires the establishment of requirements to ensure compliance with 10 CFR 50, Appendix.B.
Contrary to the above, there are no proceduces or instructions in project documents for reporting procedural violations, which include inspection hold points not verified.
Tnis is a Severity Level V violation.

Pursuant to the provisions of 10 CFR 2.201, Houston Li.ghting & Power Company is hereby required to submit to this office within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including for each violation:
(1) the reason for the violation if admitted, (2) the corrective steps which have been taken and the results achieved, (3) the corrective steps which will be taken to avoid further violations, and (4) the date when full compliance will ca achieved. Where good cause is shown, consideration will be given to extending the response time.
