r Ws NIAGARA M0 HAWK POWER CORPORATION NINE MILE POINT - UNIT 2 DOCKET NO. 50-410 Final Report for a Problem Concerning the Use of Topaz Inverters (55(e)-84-52)
Description of the Problem The General Electric Company reported in accordance with 10CFR Part 21, a defect in the Topaz inverters.
The defect was the adjustment of the low voltage shutoff and turn on for General Electric dedicated inverters which wem identified as being improperly set.
At Nine Mile Point Unit #2, there are three panels that were pmvided with Topaz inverters.
Control Room panels H13-P618 and H13-P629 initially had Topaz inverters.
The High Pressure Core Spray system utilizes a Topaz inverter located in panel H13-P625.
Analysis of Safety Implications Failure of the Topaz inverters to operate within.specified ranges of anticipated bus voltages could have adversely affected the operation of safety related systems.
Therefore, had this condition gone uncorrected, it could have adversely affected the safety of operation of the plant.
Corrective Action Since the Control Room panels H13-P618 and H13-P629 have now been provided with an uninterrupted power supply, the Topaz inverters will be removed and retained as spares.
All three inverters in question will be modified to assure proper operation at anticipated bus voltage.
The modification will be documented and dispositioned in accordance with Field Disposition-Instruction "TWDS", Revision 0.
