NRC INSPECTION REPORT 50-346/97003; NOTICE OF VIOLATION

The NRC completed an inspection at your Davis-Besse site with preliminary results
i:
presented to members of ycur staff on March 3,1997. The final results were discussed
l
with your staff on March 7,1997. The enclosed report presents the results of that
inspection.

We noted that operations personnel were, overall, knowledgeable of plant and equipment
i
status, and that this information was appropriately communicated to oncoming shifts. The
}
February 12, reactor downpower was conducted in a controlled, well managed manner.
However, two violations of NRC requirements were identified during this inspection. The
i
first violation involved three examples where approved plant procedures or maintenance
work instructions were not followed as written. As noted in the previous NRC inspection
report (50-346/96014), procedural adherence violations continue to be identified at Davis-
Besse. Your response to this violation should include a discussion of your corrective

actions beyond these specific examples, as this appears to be a recurring problem.

The second violation involved a failure to perform a technical specification required
- 1
surveillance on the decay heat removal watertight enclosure following its opening.

Contributing to this violation was a 1985 facility change request (FCR) and its associated
.
safety evaluation that delineated specific requirements related to testing of the enclosure.
I
Those requirements were not adequately addressed during the implementation of the FCR
to assure the technical specifications were revised and/o-
int procedures appropriately
updated.
The two violations are cited in the enclosed Notice of Violation, and the circumstances
surrounding the violations are described in detailin the enclosed report. Please note that
you are required to respond to this letter and should follow the instructions specified in the
enclosed Notice when preparing your response. The NRC will use your response, in part,
to determine whether further enforcement action is necessary to ensure compliance with
regulatory requirements.
