Gentlemen:

Subject: Special Inspection 50-213/85-20 (September 18-23,1985)

This inspection at the Haddam Neck Plant reviewed matters related to component

malfunctions and design deficiencies in the automatic auxiliary feedwater actua-

tion system which were identified during testing at the site on September 10,

1985. The findings were discussed with Messrs. R. Graves, G. Pitman and R.

Schmidt of your staff.

It appears that incident to NRC mandated upgrading of the auxiliary feedwater

(AFW) system, four solenoid-operated air valves required for automatic

actuation of AFW were miswired. In this case, no compromise of AFW function or

of safety channel independence has been identified. We are nonetheless

concerned about this matter as a general indicator of inadequate installation,

inadequate quality control overview, and inadequate component testing. We are

also concerned that your design change review program conducted in response to

the December 13, 1984 NRC order modifying the Haddam Neck license, does not

appear to have addressed component testing adequacy as an essential element of

design change control. As a result, it appears, as of September 23, 1985, that

your corrective actions do not provide adequate assurance that other similar

errors do not exist in your facility.

To discuss these matters further, we have scheduled a management meeting at the

NRC Region I office at 10:30 a.m. on October 31, 1985. At that meeting, please

be prepared to discuss the problems identified with the AFW system, the above

mentioned NRC concerns, and your completed and planned actions. After our

evaluation of your input during that meeting, we will inform you in writing of

our planned action on this matter. -l
