Pursuant to the Atomic Energy Act of 1954, as amended, the Energy iteorganization Act of 1974 (Public Law 93-438), and Title 10, Code of Federal itegulations. Chapter 1, Parts 30, 31, 32. 33, 34, 35, 36, 40 and 70, and in reliance on statements and representations heretofore made by the licensee, a license is hereby issued authorizing the licensee to receive, acquire, possess, and transfer byproduct, source, and special nuclear material designated below; to use such material for the purpose (s) and at the placels) designated below; to deliver or transfer such material to persons authorized to receive it in accordance with the regulations of the applicable Partish and to import such byproduct and source material. - Fete Bacchetti, Jack Ellis, James Addington, or William Bnke.
- 13. A. (1) Each seald source containing licensed material, other than ifydrogen 3, with a half-life greater than thirty days and in any form other than gas shall be
! testod for leakago md/or contanination at intervals not to exceed six months. In the absence of a certificato from a transferor indicating that a test has been mcdo within six nonths prior to the transfer, a sealed source received from anotler verton shall not be put into use until tested.
(2) Notwithstanding the periodic loak test required by this corxlition, any licensed scaled source is exangt from strh leak testa v/.ien the source contains 100 microcuries or less of beta aM/or gamma enittinc; material or 10 microcuries or less of altha anittirn material.
(3) We ceriodic leak test recpired by this condition does not apply to aealed sourcos that are stored and not being used. Se sources excepted from this test diall be tested for leakage glor to any use or transfer to anotler person taless they have been leak tested within six months prior to the date of uso or transfer.
- 9. Se test diall be capable of detecting the presence of n.005 microcuric of ra91oactive material on the test smple. We test saaple shall be taken fraa the sealed sourco or from the surfaces of the device in Wiich the sealed l source is permanently mounted or stored 'on W1ich one might expect contanination to accunulate. Records of leak test results shall be kept in mits of microcuries and maintaine for inspection by the cormission.
C. If the test reveals the gesence of 0.005 microcurie or more of removabic
- contanination, the licensee shall innediately withdraw the sealed source from use and shall cause it to be decontaminated and repaired or to be disposed of in accordance with commission regulations. A report shall be filed within 5 days of the test with Pagion III, U. S. thelear Regulatory Comnission, Office of Inspection and anforcenent, 799 Poosevelt Tbad, Glen Ellyn, Illinois 60137, describing the equipnent involved, the test results, and the corrective action taken. l l

D. Tests for leakage and/or contamination shall be perfonned by the licensee or by other persons specifically authorized by the Omnission or an h3reement State to perform such services.

- 14. Sealed sources containirvy licensed material shall not be opened or rancvei from their respective source holders by the licensee.
- 15. Installation, relocation, maintenance, repair, and initial radiation survey of devices containing licensed material and leak testing, installation, i replacement, and distmaal of sealed sources containing licensed material used in '
devices shall be performed only by Pete Bacchetti, Jack Ellis, Jaroes Addington or t1111m anke or by other persons specifically authorized by the omaission or an AJ recrient State to perfonn such services.
16 cxcept for plutonfun contained in a medical device designed for individual hur.an application, no plutoniun, rajardless of fonn, shall be delivered to a carrier for shipnent by air transport or transported in an aircraft by tre licensee.
- 17. Except as specifically providad otherwise by this license, the licensee
- shall possess and use licensed material described in Items 6, 7, and 8 of this license in accordance with statanents, representations, and grocedures contajned in applications datod April 28, 1975 and June 24, 1976, and letters dated July 16, 1975, August 3,1976 and May 22, 1970.
