Sunnary On June 25, 1985 Houston Lighting & Power (HL&P) notified Region IV of the NRC of a potentially reportable deficiency involving findings by HL&P's Pre-CAT Verification Team (PCVT) on the 125 VDC and 4 kV AC electrical systems. A total of forty-two (42) record packages were reviewed and each of them were deficient in one or more areas. Therefore, there is a possibility that a breakdown exists in the Quality Assurance program.

II.

Description of Deficiency HL&P has established a PCVT to walkdown systems and inspect records in preparation for the NRC Construction Audit Team.

The PCVT perfomed a review of both the hardware and records associated with the 125 VOC and the 4 kV AC Class IE systems. Although minor hardware discrepancies were discovered, HL&P has determined them to be not reportable.

A total of forty-two (42) documentation (record) packages which were filed within the Records Management System as completed documents were reviewed including pull cards, termination cards and inspection reports. All 42 packages exhibited one or more discrepancies including items such as the following:

Witness points on terminations were observed to be stamped-off by QC from 3 to 46 days after the temination was actually completed.

Some termination cards are filed with no QC signoff to indicate that inspections were performed.

The majority of packages reviewed displayed no reason for rework although records indicated that rework had been performed.

Line-throughs for corrections on termination cards are not being initialed and dated.

The various deficiencies were documented on a Standard Deficiency Report (50R).
