Describe transients of 11/22/85 and 11/23/85.

2. - Operator understanding of interaction between MFW startup and low load

-valves.

.3. Operator understanding of EFIC logic (e.g., need to reset EFIC to close

-AFW pump steam admission valve).

-4. Securing AFW after initiation

a. Did operators assure themselves that AFW was not required prior to securing pumps?

b.

-Would EFIC/AFW have controlled the 11/22 transient? Should AFW have been secured so quickly, or at all? Do plant procedures cover this action?

If AFW should be secured, should it be done by tripping pumps or by manipulating the control valves?

5. Root cause of PORV problem.
6. Root cause of-spurious opening of AFW steam admission valve after reactor trip. Can we be certain this valve is operable?
7. Expected overshoot of OTSG 1evel on AFW fill.
8. EFIC transmitter sensitivity.
9. Extent of documentation of post-trip review.

Criteria for inter-disciplinary post-trip review (G.L. 83-28, Item 1.1).

11.

Extent of consideration of items 1-8 in restart decision.

12. Information availability to NRC and post-trip review.
13. Should MFW startup valve have been repaired earlier? Before restart?
14. Should PORV have been repaired prior to restart?

15.

Effect on AFW reliability of review of recent transients involving EFIC ^
