During *,he'NRC inspection conducted during December 13-15, 1984, a violation of NRC r 'quirements was identified.
The vid}ation involved control of valve
- lineups during the Integrated Leakage Rate Test.
In accordance with the
" General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part.2, Appendix C (1985), the violation is listed below:
Failure to Follow the ILRT Procedure s
10 CFR;50, Appendix B, Criterion V requires that activities affecting quality be accomplished in accordance with documented instructions.
Engineering Test Procedure No. 1309.09, " Integrated Leak Rate Test," Test Step No. 7.3.6 states, " Verify any valves, which were checked or repaired, were returned to their position per (test) Appendix B, or any exceptions listed in (test) Appendix J."
Contrary to the above, on December 14, 1985, three valves (Nos. IA-15, SA-45, and NH,-1021) were fcund tagged and positioned in opposition to the signed procedural valve lineup control room test log.
This is a Severity Level IV violation (Supplement I.D.)(50-313/8436-01).
We have reviewed the actions taken prior to-the issuance of this Notice of Violation and findthem responsive to the concerns raised. We reviewed the implementation ~of'your corrective actions during the inspection of the Unit 2 Containment Integrity Leakage Rate Test during Apr;il 28-30, 1985, and determitted that full compliance had been achieved and will be maintained by those actions taken in response to the. inspectors concerns expressed during the exit interview on December 15, 1984.
No further response from you.concerning this violation is required at this time.
