APPENDIX A

NOTICE OF VIOLATION

Gulf States Utilities Docket: 50-458/85-51

River Bend Station Permit: CPPR-145

During an NRC inspection conducted on July 16, 1985, a violation of NRC

requirements was identified. The violation involved the failure to follow an

administrative procedure for control and use of procedures. In accordance with

the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10

CFR Part 2, Appendix C (1985), the violation is listed below:

Failure to Follow an Administrative Procedure

10 CFR Part 50, Appendix B, Criterion V, and the licensee's approved

quality assurance program, require that activities affecting the quality

of safety-related systems and components be conducted in accordance with

approved instructions, procedures, and drawings.

Administrative Procedure ADM-003, " Development. Control and Use of

Procedures," Revision 6, requires that the snift clerk stamp the original

Temporary Change Notice (TCN) form and both copies "TCN SHIFT SUPERVISOR

APPROVED COPY" in red ink, that the initiator retain one copy of the TCN

for the continuation of the work activity and that in the event a procedure

cannot be followed exactly, the task or evolution shall not be conducted

unless required by an emergency or casualty situation, until the procedure

has been revised.

Contrary to the above, on July 16, 1985, .ne TCN 85-1131 for procedure

FHP-0005, " Fuel Transfer Tube Operations," used for conduct of fuel

transfer operations was not stamped "TCN SHIFT SUPERVISOR APPROVED COPY"

in red ink as required,'and the fuel transfer evolution was being conducted

even-though the TCN was not followed exactly. Examples of this failure

to follow the TCN exactly included:

1. Attachment 1 and 2 for the required electrical lineup and control

board lineup were not completed and approved.

2. There were 3 examples on page 25 and 11 examples on page 26 of the

TCN where indications were crossed out and changed in red with no

procedure revision being made.

This is a Severity Level IV violation (Supplement I.D.)(458/8551-01).

Pursuant to the provisions of 10 CFR 2.201, Gulf States Utilities is hereby

required to submit to this Office within 30 days of the date of the letter

transmitting this Notice, a written statement or explanation in reply, including

for each violation: (1) the reason for the violations if admitted, (2) the

corrective steps which have been taken and the results achieved, (3) the

. corrective' steps which will.be taken to avoid further. violations, and (4) the

-date when full compliance will be achieved. Where good cause is shown,-

consideration will-be given~to extending the response time.
