-t ENCLOSURE 1 go RESPONSE TO NOTICE OF VIOLATION INSPECTION REPORT 85-37 a

ADMISSION OR DENIAL ~OF THE ALLEGED VIOLATION South Carolina Electric and Gas Company (SCE&G) is in agreement with the alleged violation as stated below and as discussed in NRC Audit Report No. 50-395/85-37.
. II.:
REASON FOR THE VIOLATION The reason for the violation is a t tr ib u t ed to three causes:
1)
Probable failure of the check valves in the air supply line to properly seat and maintain the air accumulator pressure for the a c t iv e s ide of the Feedwater Isolation
. Va lv e following isolation of air to the va lv e.
2)
Failure to addres s adequately isolation of the air supply to the-Feedwater Isolation Va lv e in the Nonconformance Notice _(NCN) disposition.

Failure to implement adequately the NCN dis position in performing.the desired isolation.
III.
CORRECTIVE STEPS TAKEN AND RESULTS ACHIEVED Following the Reactor Trip on September 20, 1985 and tha failure.of the Feedwater Is olation Valve to close on a Feedwater Isolation Signa l, immediate action was taken to restore instrument air to the valve and close it.-
The h yd r aulic leak on the in ac t iv e s id e o f the va lv e was repaired; the valve was stroke tested and returned to service prior to plant restart.
The Licensee evaluated the performance of the check va lv es in the1 instrument air supply lines with respect to NRC Information Notice 85-35.
An interim plan of action was
. established.to address va lv e operability until the check valves could be replaced.
This evaluation included justification fur continued operation if the f o llowin g conditions were met:

IV.
CORRECTIVE ACTION'TAKEN TO AVOID FURTHER VIOLATION The-Licensee's. Technical Services personne l h ave been instructed-to include clear and concise instructions wit h in NCN dispositions wh ic h specify unique or "one time" operator actions to place components, devices, systems, etc. in an ac :eptable configuration to sa tis f y des ign basis requirements.
Th is instruction will_ be included in.the appropriate procedure governing Technical Services activities with respect to NCN dispositions.
The check' valves located in the air supply l in e s to the Feedwater Isolation Va lv e s are to be replaced during the
-current re fueling ou tage.
In: addition, a modification has been generated to add-pressure gauges to directly monitor air reservoir pressure for the Feedwater Is ola tion Va lves.
This modification is planned for implementation d ur ing the current-(
r e fu e lin g outage.

Training will be conducted for the appropriate' personnel to address limitations controls, and interfaces for programs g ov erning mod i fica t ions, maintenance a c t iv i t ie s, and nonconformance notices.
Emphasis will be' placed on each group's res pons ibility and ac c ou n t ab i lit y in implementing their. res pec t ive portions of each program.

All corrective action with the exception of personnel training-should be complete prio. to-January 1,
