NRC INTEGRATED INSPECTION REPORT 50-302/97-01
Dear Mr.. Anderson:
'On February 22. 1997, the NRC completed an inspection at your Crystal River 3
reactor facility. .The en_ closed report presents the results of that
inspection.
During the six week period covered by this inspection period, your conduct of
activities at your Crystal River facility was generally characterized as
adequate.- We are concerned, however, about five violations of NRC
requirements that were identified.
In the area of Operations, two violations
were identified: one violation for clearance tagging requirements, which were
- inadequate to preclude personnel and equipment hazards and resulted in a valve
i
being repositioned while under a red tag clearance, and one violation for
failure to follow procedures, resulting in an inadvertent emergency diesel
generator start.
In the area of Maintenance, one violation was cited for
,
failure to perform a surveillance for the spent fuel pool level.
In the area

of, Engineering, two violations were cited: one violation for instrument
!
setpoint calculation assumptions not being translated into procedures and one
violation for inadequate corrective actions related to cable ampacity,

These violations are cited in the enclosed Notice of Violation, and the
circumstances surrounding the violations are described in detail in the
enclosed report.
Please note that you are required to respond to this letter
and should follow the instructions specified in the enclosed Notice when
preparing your response. The NRC will use your response, in part, to
' determine whether further enforcement action is necessary to ensure compliance
I
with regulatory requirements.
In addition, the enclosed Inspection Report identifies activities that

violated NRC requirements that will not be subject to enforcement ation
l
because the licensee's efforts in identifying and/or correcting tt violations
!
meet the riteria specified in Section VII.B. of the Enforcement Policy.

Thereforo, no response with respect to these matters is required.
