APPENDIX A NOTICE OF VIOLATION Arkansas Power & Light Company Docket:
50-368/85-22 Arkansas Nuclear One, Unit 2 License:
NPF-6 During an NRC inspection conducted during the period July 1-31, 1985, a violation of the NRC requirements was identified.
The violation involved the failure to follow the requirements of a document distribution procedure.
In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1985), the violation is listed below:
Criterion V of 10 CFR Part 50, Appendix B states, in part,
" Activities affecting quality... shall be accomplished in accordance with... procedures."
The Quality Assurance Manual-Operations (APL-TOP-1A) states,
"... the AP&L quality program also complies with the following American National Standard Institute (ANSI) standards..."
The list of ANSI standards includes N18.7-1976, " Administrative Controls and Quality Assurance for the Operational Phase of Nuclear Power Plants."
Section 5.2.15 of N18.7-1976 states, in part, "... Participating organizations shall have procedures for control of the documents and changes thereto to preclude the possibility or use of outdated or inappropriate documents."
Station Administrative Procedure 1000.04, " Procedural Program Pequirements," had been implemented in accordance with N18.7-1976.
Section 6.8.2.A of Procedure 1000.04 states, " Procedures should be distributed to the control room and placed in the plant manual within one working day following the effective date."
Contrary tc the above, the-NRC inspector noted on July 17, 1985, that a page of the controlled copy of Procedure 2202.33 posted on the Unit 2 remote shutdown panel had been deleted on April 16, 1985.
This is a Severity Level V Violation.

. Pursuant to the provisions of 10 CFR Part 2.201, Arkansas Power and Light Company is hereby required to submit to this office within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in
-reply, including for each violation:
(1) the reason for the violation if admitted, (2)'the corrective steps which have been taken and the results achieved, (3) the corrective steps which will be taken to avoid further violations, and (4) the date when full compliance will be achieved.
Where good cause is shown, consideration will be given to extending the response i
time.
