Dear Mr. Collins:

IE Bulletin 82-02 Degradation of Threaded Fasteners in the Reactor Coolant Pressure Boundary (RCPB) of PWR plants In response to Item 5 of Reference 2 above, the Omaha Public Power District supplied information (1) identifying those bolted closures of the RCPB that have experienced leakage, (2) describing the inspections made and corrective measures taken to eliminate any leakage problem, (3) indicating whether such leakage was attributed to gasket failure or design, (4) identifying closures i

and connections where fastener lubricants and injection sealant materials have been used, (5) reporting on plant experience with the application of these sealant materials and lubricants, and (6) describing the types and composition of the sealant materials and lubricants used (Reference 3).

Subsequent to that submi ttal,

an inspection relating to the District's response to IE Bulletin 82-02 was conducted during the week of May 7-11, 1984.

As a result of the inspection, the District became aware that the response to IE Bulletin 82-02, Item 5, was incomplete.

In order to complete the response, the District committed in (Reference 4) to investigate, "for the time period beginning with the start of commercial o)eration of Fort Calhoun Station up to the present, the types of lubricants wh:ch have been or could have been used on bolted closures of the RCPB components".

First, a re-verification that the correct ' bolted closures were identified and evaluated (Attachment 1).

The information in Attachment 1 is intended to augment that provided in the District's response to the IE Bulletin 82-02, Item 5, request for identi-fication of bolted closures of the RCPB which had experienced leakage (Refer-ence 3).

Secondly, the types of lubricants and sealants that have been or could have been used on these closures were investigated.

This information is provided in Attachment 2.

This information is intended to only supercede that portion of Reference 3 detailing the types of lubricants and sealants used, i

and to identify closures added since 1982 whose fasteners should be examined in accordance with IE Bulletin 82-02.

The District believes that this package represents as complete and accurate a listing as is possible of the lubricants which have been used on the fasteners of the components subject to IE Bulletin 82-02.

ATTACl# U T 1 A close re-examination of the Bulletin was made to ensure that all components covered by scope of action identified on page 3 of the Bulletin had been considered. We believe that the list of components identified in our original submittal, LIC-82-269, was complete except for the reactor vessel incore instrument flanges. These have been found to be attached to vessel head penetration piping which is six inch nominal diameter and are, thus, subject to the Bulletin. Even though the incore instrument flanges were not speci-fically identified in our original submittal, the Fort Calhoun staff did, at that time, incorporate the examination requirements of Bulletin 82-02 into the Station Maintenance Procedures.

In our review we found instances where components were included in-our pre-vious submittal but were outside the scope of the Bulletin. The following summarizes this reconsideration of scope.

Reactor Yessel The original submittal considered the control element drive mechanism autoclave flanges.Bulletin 82-02, however, states that "CRD's having an Omega Seal Weld design are excluded from this bulletin action." Our CRD's do attach to the vessel head penetration pipe flange with an Omega Seal Weld flange. Therefore CRD's would not need to be included in our original submittal.

Since they were included, and since they have had a history of some fastener corrosion due to leakage, they have been included in this re-eval uation.

As previously discussed, the incore instrument flange bolting has been included in this evaluation of lubricants used. Again, the examination requirements of the bulletin were incorporated in the Station Maintenance Procedures.

Subsequent to our initial submittal (Reference (3)), during the 1983 refueling

outage, an addition was made to the incore instrumentation pressure bound-aries. This was in the form of pipe nozzles and bayloc clamp type joints.

The nozzles were in turn welded to the upper incore instrument flanges. The Grayloc clamps are retained by four bolts each. An approved lubricant was specified for use on these fasteners at the time of initial installation.

During the 1984 refueling outage, these Grayloc fittings were disassembled and reassembled, again using an approved maintenance procedure specifying an approved lubricant. At that time, no visual or surface examinations were performed because the fasteners were not originally considered to be subject

. to the requirements of I.E.Bulletin 82-02. As a result of this reanalysis of I.E.Bulletin 82-02, the District now believes that the Grayloc fasteners should be subject to.the examination requirements of the Bulletin.

Steps are being taken to update the appropriate maintenance procedures to include visual and surface examinations. These changes will be completed prior to further maintenance activity on these joints.

Reactor Vessel (Continued)

During the 1984 refueling outage, Grayloc joints were initially installed on two spare CEDM housings for use with heated junction thermocouples. The assembly maintenance procedure requires the use of an apprcved lubricant as required by I.E.Bulletin 82-02. Since these joints have never been dis-assembled, there was no visual or surface examination of the fasteners.

Prior to disassembly of these joints, a procedure will be utilized which specifies visual and surface examination.

Steam Generators The original submittal included consideration of the primary manways, the secondary manways, and the secondary hand-holes. The bulletin specifically applies to reactor coolant pressure boundaries and clearly does not apply to these secondary manways and hand-holes. Only the primary manway fasteners are considered in this re-evaluation.

Pressurizer The original submittal included the pressurizer manway cover bolting. This is appropriate and is addressed in this re-evaluation.

The original submittal also included the pressurizer safety valves, RC-141 and RC-142. While the inlet flange mating each valve to the reactor coolant pressure boundary piping is only a nominal 3 inch flange, the flange bolting is included in tSis re-evaluation due to the District's concern for these components. They are removed and reinstalled after testing and resetting each refueling outage and the bulletin required examinations will continue to be performed on this flange bolting.

RC Pumps 3A, 3B, 3C, and 3D The original submittal included the reactor coolant pumps cover-to-case bolting. This is appropriate and is addressed in this re-evaluation.

Safety Injection Loop Check Valves Valves SI-194, 197, 200, 203, 207, 208, 211, 212, 215, 216, 219 and SI-220 were included in the District's original submittal. Only four of these valves, SI-208, 212, 216 and 220 attach directly with the reactor coolant pressure boundary piping welded connection. The so-called " bonnet" of these valves (SI-208, 212, 216 and 220) is a four bolt cover which compresses a deformable metal seal ring against the valve case. This bolted pressure boundary for each of the four (4) valves is not normally subjected to full operating RC system pressure. Ho maintenance orders could be found which dealt with maintenance work on these valves. According to the available records none of the twelve valves mentioned above have over been disassembled at the Fort Calhoun Station.

Summary In our review of maintenance history of those items specified in I.E.Bulletin 82-02, the District has found no evidence of bolting problems of the type described in the Bulletin which could be attributed to the use of lubricants or sealants.

ATTACHENT 2 Review of Manufacturer's Technical Manuals The Combustion Engineering Reactor Vessel Manual and Reactor Vessel Internals Manual were reviewed for specific lubricants to be used on the CEDM and inc~ ore instrument flange bolting. None was specified for these particular fasteners; however, a statement was found in the RV Internals Manual stating "Before 1

torquing, all bolts should be lubricated with two coats of neolube..."

The Combustion Engineering Steam Generator Instruction Manual specifies the use of " Super Moly No. 40240" to lubricate the primary manway cover bolts.

The Combustion Engineering Pressurizer Instruction Manual states, " lubricate the thread of the cover studs with a high temperature graphite and oil lubricant (50-50 by volume)..." in the instructions on the replacement of the-manway cover.

The Crosby Valve and Gage Co. Technical Manual for the pressurizer safety valves, RC-141 and RC-142, did not specify a particular lubricant for the flange bolts.

The Byron Jackson RC Pump Manual, in reference to the cover-to-case bolting states, " Prior to assembly of external fasteners... lubricate these pieces with Molykote "G" (Dow Corning Corp.) or Hi-Temp C-5A Anti-Seize (Fel Pro, Inc.) only, being certain that neither of these lubricants is permitted to come into contact with surfaces that may be wetted by reactor coolant."

Technical Manuals could not be found for the safety injection loop check valves.

Combustion Engineering Manual CEND-353, Rev. 6., was examined for acceptable lubricants. This manual is the Field Handling, Maintenance and Storage Requirements for Combustion Engineering Furnished Equiment.

Tables 5.3 and 5.4 were adopted by the Fort Calhoun Station in Administrative Procedure A-G-26A, Quality Control Program.

It is noted that, as previously stated, the Byron Jackson RC pump manual specifies the use of either Molycote "G" or Fel Pro Hi-Temp C-5A Anti-Seize. Only the Molycote "G" is found on Combustion Engineering's approved lubricant lists. Either lubricant could

~ have been used on the initial assembly of the RC pumps. The RC pump manual was written before the CEND-353, Rev. 6 was written. The CEND-353, Rev. 6 was written in August,1977..Regardless of which lubricant was used, in the 1980 Refueling Outage, visual and surface examinations were performed on all studs of the three disassembled pumps prior to their reinsta11ation. No evidence of stress corrosion cracking or. any other defect could be found which could be attributed to the use of improper lubricants or sealants. No record was made at that time of which lubricant wast used for reassembly of the pumps. The last time the pump covers were removed was during the 1984 Refueling Outage.

' All four pumps were partly disassembled to allow installation of new gaskets.

Visual and surface examinations were performed on all studs in accordance with approved maintenance procedurcs. No defects were found which could be attributed to the use of improper lubricants or sealants. The pumps were reassembled using approved maintenance procedures which specified approved lubricants.

,aintenance Records-M The attached table (Attachment 3) details the Maintenance Procedures and Special Procedures and all available revisions examined for lubricants speci-fied for use on f asteners.

It is not certain that all of the lubricants listed have been used on the subject fasteners, only that they were listed as available for use in the procedures. Sometimes a lubricant was listed only in the " Equipment Checklist" of a given proce' dure and not specifically called for in any of the actual procedure instructions.

Maintenance Orders on each component within the scope of this evaluation were examined as far back as the start of commercial operation in September, 1973.

Most of these M0's referenced an established Maintenance Procedure or Special Procedure for lubricants to be used. The following exceptions were found:

MO-19608, dated 3-28-83, provided for the application of a lubricant as a protective coating on RC 3A pump cover-to-case bolting. The M0 specified the use of " Pure Nickel Never Seez 165" but indicates that it was not adhering well'and Fel-Pro N5000 was also applied. Both are on CE's Unrestricted Lubricants list (Table 5.3).
