Gentlemen:

Subject: Inspection No. 50-293/85-31

This refers to the routine safety inspection conducted by Dr. M. McBride of this

office on November 5, 1985 to December 6, 1985 at the Pilgrim Nuclear Power Station,

Plymouth, Massachusetts, of activities authorized by NRC License No. DPR-35 and

to the discussions of our findings held by Dr. M. McBride with Mr. C. Mathis and

other members of your staff periodically during the inspection and at the conclu-

sion of the inspection.

Areas examined during this inspection are described in the NRC Region I Inspection

Report which is enclosed with this letter. Within these areas, the inspection

consisted of selective examinations of procedures and representative records, in-

terviews with personnel, and observations by the inspector.

Within the scope of this inspection, no violations were identified. However,

numerous errors were found in the tagging and listing of the valves in the RHR

system. While no valve position errors occurred and the tagging and listing errors

were of minor significance in that it is unlikely to directly affect plant safety,

the continued existence of incorrect operating procedure information, especially

following completion of the Procedure Update Program, represents a problem. In

light of the apparent tight staffing in the operations department, we question why

other means have not been made available to correct such lingering problems. We

urge you to take action to ensure that operating information is accurate, and we

will continue to review its accuracy in future inspections.
