'l-15 -9 5 REVIEW 1.0 PURPOSE This procedure describes the testing program and practices employed at the FSV Nuclear Generating Station for safety-related items.
2.0 APPLICABILITY This procedure applies to all activities involved in the designing, prescribing, scheduling, performing and verifying of tests required to demonstrate the conformance of safety-related items to specified requirements.

3.0 GENERAL REQUIREMENTS l
3.1 Testing practices employed on safety-related items are required to assure that:
l 3.1.1 All structures, systems and components; whether maintained, modi fi ed,
- repaired, reworked or replaced; perform in accordance with expected requirements.
I 3.1.2 All testing is performed in accordance with written procedures incorporating requirements, and acceptance limits specified by design or other appropriate documents.
l 3.1.3 The program provides for, as appropriate, proof tests, pre-operational tests, operational tests, l
and special tests.

3.1.4 Test procedures provide for assuring that all l
testing prerequisites have been
- met, that l
adequate test instrumentation is used and that a test is performed under suitable environmental conditions.
l 3.1.5 Test results are documented and evaluated by l
qualified individuals to assure test requirements have been satisfied.
l 3.2 SURVEILLANCE TESTING PROGRAMS l
Surveillance testing
- programs, as described in l
administrative procedures SMAP-1 and SMAP-2, allow for l
Technical Specification and Non-Technical Specification l
surveillance testing to be performed in accordance with l
approved Surveillance Test procedures.
l 3.3 ROUTINE OR NONROUTINE MAINTENANCE
- Tests, as appropriate, are performed by maintenance l
organizations during the course of the work being l
performed.
Such tests are prescribed in maintenance or surveillance procedures or are. scheduled as described by l
Procedure P-7.
These tests are reviewed by QA Services to (n) l identify and specify any appropriate test witnessing or

data verification requirements.
l 3.4 TESTS REQUIRED BY MODIFICATIONS l
3.4.1 The need for Cold Checkout Tests (CCT's),
l Functional Tests (FT's), or Special Tests (T-Tests) l relating to modification work, is identified by Technical Services and verified by the Work Review Committee.
Technical Services coordinates the l
preparation of CCT's, FT's, and T-Tests with appropriate organizations.
l 3.4.2 Tests required as a conseoiti-of modifications are identified and speciM H NED in Change Notice (CN) documents.
l 3.4.3 CN's are reviewed by QA Services to verify incorporation of appropriate testing requirements.
l 3.4.4 Controlled Work Procedures (CWP), developed to l
facilitate implementation of a CN, are reviewed by QA Services to identify and specify any appropriate test witnessing or data verification requirements, iL/

3.4.5 Completed CN documentation reviews by QA Services verify performance and acceptance of all required tests.
l 3.5 NONROUTINE OR SPECIAL TEST Tests not ccecred by paragraphs 3.2, 3.3 and 3.4 are occassiord iy determined to be necessary. Such tests may be appropriate to verify equipment or component conformance to requirements following rework / repair, to perform a failure analysis to determine cause of a failure, or to determine the extent of performance deterioration of in-service equipment or components.
l 4.0 PROCEDURE Tests required by Modification and Non Routine or Special Tests are accomplished as follows:

Appropriate clearance and/or Radiation Work Permits have been provided, if required.

- 16. Assembles required test equipment, special tools and items specified by the test procedure.

- 17. Conducts test in accordance with the procedure requirements.
Assures that all procedure spaces or data sheet blanks are properly checked or explained and required signatures, initials, dates and data are obtained for each page of the procedure.

- 18. Assures that " Procedure Deviation l
Reports" are submitted for PORC l
Review within 14 days of their l
approval.
l 19.
Promptly informs Shift Supervisor and Department Supervisor of any deficiencies found during the test.

- 20. Prepares a Station Service Request in accordance with Procedure P-7, as appropriate for any deficiencies noted.

- 22. Compares test results with specified acceptance criteria and determines acceptability of tested item.
