This refers to the inspection conducted by Messrs. J. G. Luehman, T. A. Flippo,
H. F. Bundy, and B. A. Breslau of this office during the period October 1
through November 30, 1985, of activities authorized by NRC Operating License
NPF-38 for Waterford Steam Electric Station, Unit 3, and to the discussions of
our findings with Mr. R. P. Barkhurst and other members of your staff at the
conclusion of the inspection.
Areas examined during the inspection included plant status, licensee event
report followup, monthly maintenance, monthly surveillance, ESF system
walkdown, routine inspection, license conditions, and wet mechanical draft
cooling tower water basin level.
Within these areas, the inspection consisted
of selective examination of procedures and representative records, interviews
with personnel, and observations by the inspectors.
These findings are
documented in the enclosed inspection report.
During this inspection, it was found that certain of your activities were in
violation of NRC requirements.
Consequently, you are required.to respond to
those violations, in writing,.in accordance with the provisions of Section 2.201
of the NRC's " Rules of Practice," Part 2, Title 10, Code of Federal Regulations.
Your response should be based on the specifics contained in the Notice of
. Violation enclosed with this letter.
However, in the case of Violation B,
please address the question of whether the calibration procedure that caused
_
the violation could have been used for the calibration of other instruments
important to safety.
Also, address the cause for the excessive number of pro-
cedure errors identified in paragraph 8 of the report and corrective actions
taken including generic actions on these issues.
One unresolved item was identified in paragraph 8 of the enclosed inspection
report.
The response directed by this letter and the accompanying Notice is not subject
to the clearance procedures of the Office of Management and Budget as required
by the Paperwork Reduction Action of 1980, PL 96-b11.
