Inspection 50-334/85-22

This refers to the routine resident safety inspection by Mr. W. Troskoski on

October 1 - 31, 1985, at the Beaver Valley Power Station, Unit 1, Shippingport,

Pennsylvanis.

The inspection consisted of document reviews, interviews and

observation of activities. The findings were discussed with Mr. J. Sieber of

your staff.

Based on the results of this inspection, it appears that certain of your ac-

tivities were not conducted in full compliance with NRC requirements, as set

forth in the Notice of Violation, enclosed herewith as Appendix A.

These

violations have been categorized by severity level in accordance with the NRC

'

Enforcement Policy (47 FR 8583) dated March 8,1984. You are required to re-

spond to this letter and in preparing your response, you should follow the

instructions in Appendix A.

,

In addition, we are concerned that Violation A is the second violation to occur

in the past 12 months involving failures to issue contractor personnel termina-

,

tion exposure reports. Your response should specifically address those actions

to prevent recurrence. Violation B was caused by contractor personnel inadver-

tently opening a valve that served as the single isolation point between a

radioactive fluid system and the environment thru a Unit 2 cross connect line.

Though the release is judged to be of minor significance, it serves to point

out a potential weakness in your program to control the interface of those

systems shared by both Unit 1 and 2, while Unit 2 is still under construction.

Consequently, your response should also address those actions taken to ensure

that all potential release paths to Unit 2 are properly blocked until such time

as the lines are required for testing or system operation.

The responses directed by this letter and the accompanying Notice are not sub-

ject to the clearance procedures of the Office of Management and Budget as

required by the Paperwork Reduction Act of 1980, PL 96-511.
