ENCLOSURE BELLEFONTE NUCLEAR PLANT UNIT 1 DEFICIENT DOCUMENTATION FOR CONDUIT FIRE STOPS IN THE UNIT 1 DIESEL GENERATOC BUILDING BLRD-50-438/85-22 NCR 4414 10 CFR 50.55(e)

FINAL REPORT Description of Deficiency Conduit fire stop sealing program initiated in the unit 1 diesel generator building was performed under the requirements of Bellefonte Nuclear Plant (BLN) Quality Control Procedure (QCP) 3.5, " Penetration Fire Stops and Pressure and Moisture Seals," and cognizance of the Electrical Engineering Unit (EEU). At the time the seals were installed, temporary identification numbers were assigned to the documentation. After the installation activity was conpleted, the Office of Engineering (OE) issued drawings depicting the official identifacation (reference SDWO816-RZ, 818-RZ series). An unsuccessful attempt was made by BLN personnel to reassign the completed documentation with permanent identification. Therefore, the actual status of the fire stop sealing program is indeterminate. This deficiency is applicable to the unit 1 diesel generator building at BLN only.

The cause of the deficiency was a failure to develop an adequate identifying and tracking program for the installation of the fire stop seals.

Safety Implications Possible deficiencies in conduit fire stop seals could allow the propogation of fire between designated fire areas and could allow contaminated smoke or gases to flow between designated pressure areas. This could potentially affect the ability to achieve and maintain the safe shutdown of the plant.

Therefore, if this condition had remained uncorrected, the safe operation of the plant could have been adversely affected.

Corrective Action BLN Office of Construction (OC) has conducted a sampling of the seals in the unit 1 diesel generator building in accordance with BLN QCP 5.18. "Firestops, Moisture, Pressure, and Radiation Seals," and Military Standard (MIL-STD) 105D to check for cell structure and fill. The sampling identified that additional actions will be required to complete the fire stop sealing program for the

.4 1-TVA's Office of Engineering (OE) has provided OC with the official ID numbers for the conduit fire stop seals in the unit 1 diesel generator building. All of the seals will be reinspected, resealed if necessary, and documented--

utilizing the new identification numbers shown on the SDWO816-RZ, 818-RZ drawing series. This action will be completed by one year before fuel load of unit 1.

To prevent recurrence, all conduit fire stop seals now have official identi-fication and are now monitored by the ECM&D (Engineering, Construction, Monitoring and Documentation) computer program, which tracks the current status of the fire stop seals.
