OI REPORT 4-94-010; RE: FALSIFICATION OF VEHICLE RADIATION SURVEY RECORDS AT FORT ST. VRAIN I

The above captioned OI report involves allegations of falsification of vehicle radiation survey records by a contractor at Ft. St. Vrain. OI substantiated that certain records contractor and radiation employees work permits. deliberately falsified radiation survey It appears that enforcement action is appropriate in this case with respect to falsification of these documents.

The OI report also discussed allegations of harassment and intimidation at Ft.

St. Vrain. This matter was investigated by the licensee's independent counsel '
