SUBJECT: NRC INSPECTION REPORT 50-313/96-09:50-368/96-09 AND NOTICE OF

VIOLATION

Dear Mr. Hutchinson: l

An NRC inspection was conducted December 22,1996, through February 1,1997, at your ,

Arkansas Nuclear One, Units 1 and 2, reactor facilities. The enclosed report presents the I

scope and results of that inspection, j

!

During the 6-week period covered by this inspection, your conduct of activities at the ANO

facility was generally characterized by safety conscious operations, sound engineering and

maintenance practices, and appropriate radiological controls. Two violations of NRC

requirements were identified. The first violation resulted from the improper clearance of a

hold card while work on a 480 volt breaker was in progress. Although no personnel were

injured, this error created the potential for a personnelinjury or fatality. Significant

weaknesses were noted in the lack of communications between the control room

supervisor and shift superintendent and the failure of an auxiliary operator to question the

work being performed by the electricians in a panel that he knew was energized. Although

this violation satisfied the criteria to be noncited pursuant to Section Vll.B.1 of the NRC's

Enforcement Policy, it is being cited due to the potential significance of the issue.

Equipment isolation and restoration programs and the implementation of these programs

are principal barriers in ensuring both personnel and nuclear safety. The second violation

resulted from your failure to update the Unit 1 Updated Safaty Analysis Report (USAR). i

These violations are cited in the enclosed Notice of Violation (Notice) and the

circumstances surrounding the violations are described in detail in the enclosed report.

Please note that you are required to ret"ond to this letter and should follow the

instructions specified in the enclosed Nace when preparing your response. The NRC will

use your response, in part, to determine whether further er.forcement action is necessary

to ensure compliance with regulatory requirements.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter,

its enclosures, and your response will be placed in the NRC Public Document Room (PDR).

To the extent possible, your response should not include any personal privacy, proprietary,

or safeguards information so that it can be placed in the PDR without redaction.
